血清炎症细胞因子检测对预警子宫内膜癌术后深静脉血栓的应用分析

Analysis of Serum Inflammatory Cytokine Detection for Early Warning of Deep Vein Thrombosis after Endometrial Carcinoma

目的 探讨血清炎症细胞因子检测对预警子宫内膜癌术后深静脉血栓(DVT)的应用分析。方法 收集2017年3月到2021年3月收治的56例子宫内膜癌手术后发生DVT患者临床资料,将其纳入DVT组,另选取同期56例术后未发生DVT患者临床资料,将其纳入非DVT组。两组术前均测定血清炎症细胞因子水平[中性粒细胞、抗肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)],并分析其检测效能。结果DVT组患者血液中性粒细胞计数高于非DVT组,差异有统计学意义(P<0.05)。DVT组血清中TNF-α、IL-6与IL-8高于非DVT组患者,差异有统计学意义(P<0.05)。多因素Logistic回归显示术前中性粒细胞计数升高与血清TNF-α浓度升高可能是子宫内膜癌患者术后DVT的危险因素。绘制受试者工作特征(ROC)曲线,中性粒细胞计数值、TNF-α单独及联合预测子宫内膜癌术后DVT的曲线下面积(AUC)分别为0.886、0.781、0.926,均有一定价值。结论 子宫内膜癌患者术前中性粒细胞、TNF-α水平上升对预测DVT的发生具有一定的预警效果,IL-6、IL-8对DVT预测无价值。

Objective To explore the value of serum cytokine detection in predicting postoperative deep venous thrombosis in endometrial cancer.Methods The clinical data of 56patients with DVT after surgery for endometrial cancer admitted to this hospital from March 2017to March 2021were selected and included in the DVT group.In the same period,the clinical data of 56patients without DVT after surgery were selected.Included in the non-DVT group.The levels of serum inflammatory cytokines were measured before operation in both groups[neutrophils,anti-tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-1β(IL-1β),Interleukin-8(IL-8)],and analyze its detection efficiency.Results Compared with patients in the non-DVT group,patients in the DVT group had significantly higher blood neutrophil counts and significantly lower hemoglobin levels.the difference was statistically significant(P<0.05).Compared with patients in the non-DVT group,the serum levels of TNF-α,IL-6and IL-8were significantly higher in the DVT group.the difference was statistically significant(P<0.05).Multivariate logistic regression showed that the increase of preoperative neutrophil count and elevated serum TNF-αconcentration may be risk factors for postoperative DVT in patients with endometrial cancer.The ROC curve predicts DVT efficacy by risk factors.The neutrophil count predictive sensitivity is 88.9% and the specificity is 76.3%.The ROC curve predicts the DVT efficacy of risk factors.The predictive sensitivity of TNF-αis 75.0% and the specificity is 76.3%.Conclusion The preoperative neutrophil and TNF-αlevels of patients with endometrial cancer have a certain early warning effect in predicting the occurrence of DVT,while IL-6and IL-8have no value in predicting DVT.
