血清UCH-L1水平与急性脑梗死的诊断及预后关系探讨 被引量:5

摘要 目的探讨血清泛素羧基末端水解酶-1(UCH-L1)表达水平与急性脑梗死(ACI)患者的诊断及预后的关系。方法选取2017年6月-2020年7月ACI诊疗的患者共计98例作为观察组。另选同时间体检的60位健康者作为对照组。检测并对比两组研究对象的UCH-L1表达水平,分析观察组患者UCH-L1表达水平与脑梗死面积、神经缺损程度的关系。与此同时,用改良Rankin量表(mRS)评估观察组患者预后情况,比较不同预后情况之间UCH-L1表达水平与有何关系。结果观察组患者UCH-L1表达水平显著高于对照组,差异有统计学意义(P<0.05)。UCHL1表达水平与患者脑梗死面积以及神经缺损程度存在正相关性(P<0.05),UCH-L1表达水平随着患者的脑梗死面积的增大以及神经缺损程度加重而升高(P<0.05)。预后情况良好者UCH-L1表达水平低于预后情况不良者(P<0.05),ROC曲线结果显示,UCH-L1表达水平临界值为0.72μg/L,预测预后不良的灵敏度为83.3%,特异性为90.2%,对预测患者预后不良情况有重要意义。结论ACI患者UCH-L1表达水平较常人显著升高,且UCH-L1表达水平与患者脑梗死面积、神经缺损程度存在正相关性,对预测患者预后情况有一定价值。 Objective To explore and discuss the relationship between the level of serum ubiquitin carboxyl terminal hydrolase-1(UCH-L1)and the diagnosis and prognosis of patients with acute cerebral infarction(ACI).Methods a total of 98patients with ACI in our hospital from June 2017to July 2020were selected as the subjects of the observation group.Another 60healthy people who came to our hospital for physical examination at the same time were selected as the control group of this study.The UCH-L1levels of the two groups were detected and compared,and the relationship between the UCH-L1level and the area of cerebral infarction and the degree of nerve defect in the observation group was evaluated.At the same time,the correlation between UCH-L1level and the prognosis of patients in the observation group was evaluated by modified Rankin Scale(MRS).Results The level of UCH-L1in the observation group was significantly higher than that in the control group(P<0.05).The expression level of UCH-L1was positively correlated with the area of cerebral infarction and the degree of nerve defect(P<0.05).The expression level of UCH-L1increased with the increase of the area of cerebral infarction and the degree of nerve defect(P<0.05).The expression level of UCH-L1in patients with good prognosis was lower than that in patients with poor prognosis(P<0.05).ROC curve showed that the critical value of UCH-L1expression level was 0.72μg/L,the sensitivity of predicting poor prognosis was 83.3%,and the specificity was 90.2%.Conclusion the expression level of UCH-L1in patients with ACI is significantly higher than that in normal people,and the expression level of UCH-L1is positively correlated with the area of cerebral infarction and the degree of nerve defect,and negatively correlated with the prognosis of patients.It can be used to reflect the condition and prognosis of patients with ACI.
