围手术期电针干预在预防股骨骨折术后老年患者谵妄的效果研究

Effect of perioperative electroacupuncture intervention on the prevention of delirium in elderly patients after femoral fracture surgery

目的探讨围手术期电针干预在预防老年股骨骨折术后谵妄中的临床价值。方法选择2021年3月—2022年1月本院收治的60例老年股骨骨折手术患者作为研究对象,采用随机数表法分为研究组(30例,接受术前电针干预)与对照组(30例,接受传统手术)两组。比较两组患者的日常生活能力评定量表(ADL)、谵妄发生率、简易精神状态评价量表(MMSE)、谵妄持续时间、匹兹堡睡眠质量指数(PSQI)、住院及卧床时间。结果手术前两组的ADL、PSQI、MMSE评分差异无统计学意义(P均>0.05),术后7天研究组的ADL、MMSE评分高于对照组,PSQI评分低于对照组,差异有统计学意义(P<0.05)。研究组的谵妄发生率(20%)低于对照组(50%),研究组的谵妄持续时间、住院时间、卧床时间均短于对照组,差异均有统计学意义(P均<0.05)。结论围手术期电针干预可降低老年股骨骨折患者术后谵妄发生风险,改善患者生活及睡眠治疗,缩短患者住院时间。

Objective To explore the clinical value of perioperative electroacupuncture intervention in the prevention of delirium after femoral fracture surgery in the elderly.Methods 60 elderly patients with femoral fracture those were admitted to our hospital from March 2021 to January 2022 were enrolled as study objects,ans they were randomly divided into study group(30 cases,receiving preoperative electroacupuncture intervention)and control group(30 cases,receiving traditional surgery).The score of activities of daily living scale(ADL),incidence of delirium,score of Mini Mental State Assessment Scale(MMSE),duration of delirium,score of Pittsburgh sleep quality index(PSQI),hospitalization and bed time were compared between the two groups.Results There was no significant difference in the scores of ADL,PSQI and MMSE between the two groups before operation(P>0.05).On the 7th day after operation,the scores of ADL and MMSE in the study group were higher than those in the control group,and the score of PSQI was lower than those in the control group(P<0.05).The incidence of delirium in the study group(20.00%vs 50.00%)was lower than that in the control group.The duration of delirium,hospital stay and bed stay in the study group were shorter than those in the control group(all P<0.05).Conclusions Perioperative electroacupuncture intervention could reduce the risk of postoperative delirium in elderly patients with femoral fracture,improve their life and sleep treatment,and shorten their hospitalization time.
