高血压患者遵医嘱依从性的影响因素分析

Analysis of Influencing Factors on Compliance with Doctor’s Advice in Hypertensive Patients

目的:探讨高血压患者遵医嘱依从性的影响因素。方法:以整群抽样方式选择2019年1月-2021年10月就诊于江西省第五人民医院的143例高血压患者。收集资料,分析高血压患者遵医嘱依从性的影响因素。结果:143例高血压患者,遵医嘱依从性好47例(32.87%),遵医嘱依从性差96例(67.13%);遵医嘱依从性差患者年龄≥60岁、月收入<2000元、文化程度初中及以下、服药种类≥2种、有药物不良反应、服药频率≥2次/d比例均高于遵医嘱依从性好患者,差异均有统计学意义(P<0.05);两者性别、病程、参加医疗保险、婚姻情况、工作状况、家族史比例对比,差异均无统计学意义(P>0.05);多因素分析显示,年龄≥60岁[OR=2.319,95%CI(1.115,4.824)]、月收入<2000元[OR=2.970,95%CI(1.443,6.111)]、文化程度初中及以下[OR=3.162,95%CI(1.502,6.658)]、服药种类≥2种[OR=4.452,95%CI(2.055,9.642)]、有药物不良反应[OR=5.850,95%CI(2.701,12.668)]、服药频率≥2次/d[OR=2.063,95%CI(1.016,4.189)]均是影响高血压患者遵医嘱依从性的高危因素(P<0.05)。结论:高血压患者遵医嘱依从性受年龄、月收入、文化程度、服药种类、药物不良反应、服药频率影响。

Objective:To investigate the influencing factors of compliance with doctor’s advice in hypertensive patients.Method:A total of 143 hypertensive patients who treated in the Fifth People’s Hospital of Jiangxi Province from January 2019 to October 2021 were selected by cluster sampling.Data were collected to analyze the influencing factors of compliance with doctor’s advice in hypertensive patients.Result:Among 143 hypertensive patients,47 cases(32.87%)had good compliance with doctor’s orders,and 96 cases(67.13%)had poor compliance with doctor’s orders.The proportion of age≥60 years old,monthly income<2000 yuan,education level of junior high school and below,types of medication≥2 kinds,adverse drug reactions and frequency of medication≥2 times/d in patients with poor compliance with doctor’s orders were higher than those in patients with good compliance with doctor’s advice,and the differences were statistically significant(P<0.05).There was no significant difference in the proportions of marital status,work status,and family history between two patients(P>0.05).Multivariate analysis showed that age≥60 years old[OR=2.319,95%CI(1.115,4.824)],monthly income<2000 yuan[OR=2.970,95%CI(1.443,6.111)],education level of junior high school and below[OR=3.162,95%CI(1.502,6.658)],types of medication≥2 kinds[OR=4.452,95%CI(2.055,9.642)],adverse drug reactions[OR=5.850,95%CI(2.701,12.668)],frequency of medication≥2 times/d[OR=2.063,95%CI(1.016,4.189)]were high risk factors affecting the compliance of hypertensive patients(P<0.05).Conclusion:The compliance of hypertensive patients with doctor’s advice is affected by age,monthly income,educational level,type of medication,adverse drug reactions and medication frequency.
