摘要 目的评价红细胞分布宽度(red blood cell distribution width,RDW)预测骨质疏松症的价值。方法搜集山东第一医科大学第二附属医院2020年4月—2021年4月采用双能X射线骨密度仪(dual‑energy X‑ray absorptiometry,DXA)测量骨密度(bone mineral density,BMD)人群的相关临床资料、BMD值、T值和RDW值,亚洲人骨质疏松自我筛查工具(osteoporosis self‑assessment tool for Asians,OSTA)评分、骨折情况以及骨折风险预测简易工具(fracture risk assessment tool,FRAX)评分等信息。运用Pearson相关分析及ROC曲线分析RDW预测骨质疏松症的能力。结果骨质疏松患者、骨量减少患者和骨量正常患者在性别、年龄、体质量指数(body mass index,BMI)、是否合并冠心病、BMD值、T值、RDW值、OSTA评分、是否合并骨折、主要骨折风险及髋部骨折风险方面,差异均具有统计学意义(P<0.05)。ROC曲线分析结果发现,RDW预测骨质疏松症的曲线下面积为0.622,最佳预测值为12.65%(P=0.003)。结论RDW具有一定预测骨质疏松症的价值,但是仍需要前瞻性、大样本的临床研究进一步证实。 Objective:To explorethe value of red blood cell distribution width(RDW)for predicting osteoporosis.Methods:The clinical data,bone mineral density(BMD)value,T value,RDW value,osteoporosis self-assessment tool for Asians(OSTA)score,fracture status and fracture risk assessment tool(FRAX)score of patients who underwent BMD measurement by dual-energy X-ray absorptiometry(DXA)in our hospital from April 2020 to April 2021 were collected.The ability of RDW in predicting osteoporosis was evaluated by Pearson's correlation analysis and ROC curve analysis.Results:There were significant differences in gender,age,body mass index(BMI),coronary heart disease,BMD value,T value,RDW value,OSTA score,major fracture risk and hip fracture risk among patients with osteoporosis,patients with reduced bone mass,and patients with normal bone mass(all P<0.05).ROC curve analysis showed that the area under the curve was 0.622,and the best predictive value was 12.65%(P=0.003).Conclusion:The value of RDW still needs to be further confirmed by prospective and large sample clinical studies in predicting osteoporosis.
