加味生化汤联合揿针促进产后子宫复旧及乳汁分泌的临床研究

Clinical Study of Modified Shenghua Decoction Combined with Pressing Acupuncture on Promoting Postpartum Uterine Involution and Milk Secretion

目的:探讨加味生化汤联合揿针促进产后子宫复旧及乳汁分泌的临床效果。方法:选取2021年1-11月在赣州市南康区中医院妇产科住院治疗的100例经阴道分娩的产妇,采用随机数字表法将其分为对照组和研究组,每组50例。对照组静滴缩宫素,研究组在对照组基础上加服加味生化汤及揿针留针。比较两组血性恶露量及结束时间、宫底下降距离、子宫收缩痛和乳房胀痛程度、乳汁分泌功能。结果:研究组产后2、3、4 d血性恶露量均少于对照组,差异均有统计学意义(P<0.05)。两组血性恶露结束时间比较,差异有统计学意义(P<0.05)。产后3、5 d,研究组乳房胀痛和子宫收缩痛情况均优于对照组,差异均有统计学意义(P<0.05)。产后1 d,两组宫底高度下降值比较,差异无统计学意义(P>0.05)。产后3、5 d,研究组宫底高度下降值均高于对照组,差异均有统计学意义(P<0.05)。研究组乳汁分泌功能优良率高于对照组,差异有统计学意义(P<0.05)。结论:加味生化汤联合揿针减少血性恶露量,减轻产后妇女疼痛,促进产后子宫复旧及乳汁分泌,具有一定的临床价值。

Objective:To investigate the clinical effect of modified Shenghua Decoction combined with pressing acupuncture on postpartum uterine involution and milk secretion.Method:A total of 100 cases of puerpera who were hospitalized in the Department of Obstetrics and Gynecology of Ganzhou Nankang District Traditional Chinese Medicine Hospital from January to November 2021 were selected,they were divided into control group and study group according to random number table method,with 50 cases in each group.The control group was treated with intravenous drip of Oxytocin,and the study group was treated with modified Shenghua Decoction combined with pressing acupuncture on the basis of the control group.Lochia rubra amount and end time,the bottom of the uterine descent distance,uterine contraction pain and breast swelling pain and milk secretion function were compared between the two groups.Result:The amount of lochia rubra in the study group were lower than those in the control group on postpartum 2,3 and 4 d,and the differences were statistically significant(P<0.05).There was significant difference in the end time of lochia rubra between two groups(P<0.05).On postpartum 3 and 5 d,breast pain and uterine contractions in the study group were better than those in the control group,the differences were statistically significant(P<0.05).On postpartum 1 d,there was no significant difference in the decrease value of fundus height between the two groups(P>0.05).On postpartum 3 and 5 d,the decrease value of fundus height in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The excellent and good rate of milk secretion in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Modified Shenghua Decoction combined with pressing acupuncture can reduce the amount of bloody lochia,relieve pain in postpartum women,and promote postpartum uterine involution and milk secretion,which has certain clinical value.
