甲状旁腺自发荧光识别技术在甲状腺及甲状旁腺手术中的应用

摘要 目的:分析探究甲状旁腺自发荧光识别技术在甲状腺及甲状旁腺手术中的应用价值。方法:选取于2020年1月~2021年2月在绵阳市第三人民医院甲状腺外科和头颈外科就诊的64例患者作为研究对象,所有患者均行甲状腺癌根治术。分步骤依次逐步探查甲状腺旁腺,分析肉眼和自体荧光检出甲状旁腺的情况。结果:64例患者中,共计检测出甲状旁腺155枚,其中单侧手术46例,检测出92例,占比约为59.35%,双侧手术18例检出甲状旁腺63例,占比约为40.65%。其中,上甲状旁腺和下甲状旁腺检出分别为80枚、75枚,各占比例约为51.61%、48.39%。肉眼检出甲状旁腺为227枚(93.0%),近红外显像仪检出241枚(98.8%),差异有统计学意义(P<0.05)。对stageⅠ~Ⅲ上甲状旁腺和下甲状旁腺进行统计分析得出,在stageⅠ阶段肉眼和自体荧光对上甲状旁腺的检出率对比无差异,而下甲状旁腺的检出率对比存在明显差异(P<0.05)。而stageⅡ和Ⅲ阶段,自体荧光对上甲状旁腺与下甲状旁腺检出率均高于肉眼检出,差异具有统计学意义(P<0.05)。结论:自发荧光识别技术检测甲状旁腺的检出率明显较高,同时能够有效的降低甲状旁腺的误切率。 Objective:To analyze and explore the application value of parathyroid autofluorescence recognition technology in thyroid and parathyroid surgery.Methods:A total of 64 patients who visited the Department of Thyroid and Head and Neck Surgery of the Third People's Hospital of Mianyang City from January 2020 to February 2021 were selected as the research subjects.All patients underwent radical thyroidectomy.The parathyroid glands were explored step by step,and the detection of parathyroid glands by naked eye and autofluorescence were analyzed.Results:A total of 155 parathyroids were detected in 64 patients,including 46 cases of unilateral surgery,92 cases,accounting for 59.35%,18 cases of bilateral surgery,63 cases,accounting for 40.65%.Among them,the detection of upper parathyroid gland and lower parathyroid gland were 80 and 75 respectively,accounting for 51.61%and 48.39%respectively.There were 227(93.0%)of parathyroid glands detected by naked eyes,and 241(98.8%)of parathyroid glands detected by near-infrared imaging,the difference was statistically significant(P<0.05).The statistical analysis of the upper parathyroid gland and the lower parathyroid gland of stageⅠ~Ⅲshowed that there was no difference in the detection rate of the upper parathyroid gland by naked eye and autofluorescence at stageⅠ,but there was a significant difference in the detection rate of the lower parathyroid gland(P<0.05).In stageⅡand stageⅢ,the detection rates of upper parathyroid gland and lower parathyroid gland by autofluorescence were higher than those by naked eyes,and the difference was statistically significant(P<0.05).Conclusion:The detection rate of parathyroid gland detected by spontaneous fluorescence recognition technology is significantly higher,and at the same time,it can effectively reduce the false rate of parathyroid glands.
