头颅MRI在急性脑干梗塞临床诊断中的作用分析

The Role of Head MRI in the Clinical Diagnosis of Acute Brainstem Infarction

目的:分析头颅MRI在急性脑干梗塞临床诊断中的作用。方法:选择2018年5月~2021年5月本院收治的86例急性脑干梗塞患者作为调查对象,86例患者均进行头颅MRI检查,分析急性脑干梗塞的临床特征。结果:患者进行病理检查后明确梗死病灶有98个,MRI检查确定梗死病灶有94个。多发梗死、单发梗死、脑桥处梗死、延髓外梗死、中老处梗死、混合梗死等检查结果相比较P>0.05,差异无统计学意义。MRI检查具有较高的诊断准确率。MRI和病理学诊断基底节、额叶、脑干、顶叶、颞叶以及小脑病灶检出率对比差异不具有统计学意义(P>0.05),根据MRI影像结果发现,在86例患者中,T_(2)WI信号较高的有41例,占比为57.67%。T_(1)WI信号相同,T_(2)WI信号凸显稍高为26例,所占比为30.23%。然而T_(1)WI信号和T_(2)WI信号混杂不匀为9例,占比为10.47%。结论:应用MRI诊断急性脑干梗塞诊断准确率较高,具有重要的临床价值。

Objective:To analyze the role of cranial MRI in the clinical diagnosis of acute brainstem infarction.Methods:A total of 86 patients with acute brainstem infarction admitted to our hospital from May 2018 to May 2021 were selected as the subjects of investigation.All 86 patients underwent head MRI examination to analyze the clinical features of acute brainstem infarction.Results:After pathological examination,98 infarct lesions were confirmed by the patient,and 94 infarct lesions were confirmed by MRI examination.The results of multiple infarction,single infarction,pontine infarction,extramedullary infarction,middle and old infarction,mixed infarction were compared with P>0.05,and the difference was not statistically significant.MRI examination has a high diagnostic accuracy rate.There was no statistically significant difference in the detection rate of MRI and pathological diagnosis of base segment,frontal lobe,brainstem,parietal lobe,temporal lobe,and cerebellar lesions(P>0.05),according to MRI imaging results,in 86 patients,T_(2)WI There were 41 cases with higher signal,accounting for 57.67%.T_(1)WI signal was equal,and T_(2)WI signal showed slightly higher in 26 cases,accounting for 30.23%.And there were 9 cases of T_(1)WI signal and T_(2)WI signal mixing unevenly,accounting for 10.47%.Conclusion:MRI diagnosis of acute brainstem infarction has a high diagnostic accuracy and has important clinical value.
