子宫切口妊娠诊断中经阴道彩色多普勒超声的应用及临床价值探讨

Application and Clinical Value of Transvaginal Color Doppler Ultrasound in the Diagnosis of Pregnancy with Uterine Incision

目的:探讨分析子宫切口妊娠诊断中经阴道彩色多普勒超声的应用及临床价值。方法:本次研究择取研究对象为2018年2月~2020年10月收治入院,经手术后病理诊断确诊的48例子宫切口妊娠患者,所有患者均同时接受经腹部彩色多普勒超声检查以及经阴道彩色多普勒超声检查,对两种超声检测方式的诊断价值进行对比分析,并对治疗后患者的子宫前壁下段血流阻力指数、子宫切口肌层厚度进行经阴道超声检测。结果:经阴道超声检测的诊断准确率、敏感性都为93.75%,相对比于诊断准确率、敏感性都为70.83%的经腹部超声检测明显较高(P<0.05);患者接受临床治疗后,经阴道超声检查显示,子宫前壁下段肌层血流阻力指数相较治疗前增高,切口肌层厚度相较治疗前增厚,存在明显差异性(P<0.05)。结论:经阴道彩色多普勒超声诊断子宫切口妊娠的准确率相较经腹部彩色多普勒超声更高,具有较高的灵敏度,同时对于子宫切口妊娠患者治疗效果也具有良好的评估价值。

Objective:To explore the application and clinical value of transvaginal color Doppler ultrasound in the diagnosis of uterine incision pregnancy.Methods:In this study,the selected subjects were admitted to hospital from February 2018 to October 2020,48 patients with uterine incision pregnancy confirmed by pathological diagnosis after operation,all patients were examined by transabdominal color Doppler ultrasound and transvaginal color Doppler ultrasound at the same time.The diagnostic value of the two ultrasound detection methods was compared and analyzed,and the blood flow resistance index of the lower segment of the anterior uterine wall and the thickness of the uterine incision muscle layer were measured by transvaginal ultrasound after treatment.Results:The diagnostic accuracy and sensitivity of transvaginal ultrasound was 93.75%,which was significantly higher than that of transabdominal ultrasound with diagnostic accuracy and sensitivity of 70.83%(P<0.05);After clinical treatment,transvaginal ultrasound examination showed that the blood flow resistance index of the lower segment of the anterior uterine wall increased compared with that before treatment,and the thickness of the incision muscle layer increased compared with that before treatment,with significant differences(P<0.05).Conclusion:In the clinical diagnosis of patients with uterine incision pregnancy,the accuracy of transvaginal color Doppler ultrasound diagnosis is higher than that of transabdominal color Doppler ultrasound,and it has higher sensitivity.At the same time,it also has a good evaluation value for the therapeutic effect of patients with uterine incision pregnancy.
