取卵术中改用双腔针患者IVF妊娠结局分析

Analysis of IVF Pregnancy Outcome in Patients with Double Chamber Needle During Oocyte Retrieval

目的:探讨取卵术中因获卵率明显低于预期由单腔取卵针改为双腔取卵针的边缘性空卵泡综合征患者体外受精-胚胎移植(IVF-ET)结局。方法:比较单腔改双腔取卵针(研究组,26例)和常规单腔取卵针(对照组,26例)患者的获卵率、正常受精胚胎数、D3优质胚胎数、可利用胚胎数、累积妊娠率、早期流产率、术中出血量及患者麻醉苏醒后疼痛评分。结果:两组患者相比研究组的获卵率(61.5%vs 99.2%,P=0.000)、正常受精胚胎数(4.27±3.66 vs 6.46±3.26个,P=0.027)、D3优质胚胎数(2.73±2.46 vs 5.00±3.58个,P=0.010)、可利用胚胎数(2.38±1.42 vs 3.42±2.14个,P=0.044)明显低于对照组,但是累积妊娠率(41.7%vs 64%,P=0.117)及早期流产率(20%vs 11.8%,P=0.613)无显著差异。术中出血量(4.0±2.15 vs 3.46±1.77mL,P=0.330)、患者麻醉苏醒后疼痛评分(2.08±1.74 vs 1.92±1.83,P=0.758)无显著差异。结论:取卵术中要及时识别边缘性空卵泡综合征患者并改用双腔取卵针以不影响患者累积妊娠率。

Objective:To investigate the outcome of in vitro fertilization embryo transfer(IVF-ET)in patients with marginal empty follicle syndrome who changed from single cavity to double cavity oocyte collection needle because the egg retrieval rate was significantly lower than expected.Methods:The oocyte retrieval rate,the number of normal fertilized embryos,the number of D3 high-quality embryos,the number of available embryos,the cumulative pregnancy rate,the early abortion rate,the amount of intraoperative bleeding and the pain score after anesthesia awakening were compared between patients with single cavity double cavity oocyte collection needle(study group,26 cases)and patients with conventional single cavity oocyte collection needle(control group,26 cases).Results:Compared with the two groups,the egg retrieval rate(61.5%vs 99.2%,P=0.000),the number of normal fertilized embryos(4.27±3.66 vs 6.46±3.26,P=0.027),the number of D3 high-quality embryos(2.73±2.46 vs 5.00±3.58,P=0.010)and the number of available embryos(2.38±1.42 vs 3.42±2.14,P=0.044)in the study group were significantly lower than those in the control group,however,there was no significant difference in cumulative pregnancy rate(41.7%vs 64%,P=0.117)and early abortion rate(20%vs 11.8%,P=0.613).There was no significant difference in the amount of intraoperative bleeding(4.0±2.15 vs 3.46±1.77mL,P=0.330)and the pain score after anesthesia Awakening(2.08±1.74 vs 1.92±1.83,P=0.758).Conclusion:The patients with marginal empty follicle syndrome should be identified in time during egg retrieval,and the double cavity egg retrieval needle should be used instead so as not to affect the cumulative pregnancy rate of the patients.
