摘要
目的:探讨幽门螺杆菌测试仪碳14尿素呼气试验(C^(14)-UBT)检测幽门螺杆菌感染临床效果。方法:纳入本单位2018年1月~2019年1月因腹部不适需行腹部检查的78例患者作为研究样本,入院后均行C^(14)-UBT和快速尿素酶试验(rapiduurease test,RUT),收集两种方法的检测结果进行比较,对比其敏感度、准确性及特异度,并对比不同消化道疾病中幽门螺旋杆菌的检出率。结果:C^(14)-UBT与RUT检测Hp阳性率分别为65.38%、62.82%;本组78例患者中,共检出幽门螺旋杆菌阳性患者50例,占比64.10%,C^(14)-UBT检测的敏感度、特异度分别为96.0%、89.29%,约登指数0.852;RUT检测的敏感度为88.0%,特异度为82.14%,约登指数0.793,无统计学差异(P>0.05);C^(14)-UBT检测胃溃疡、萎缩性胃炎、十二指肠溃疡、胃癌的Hp阳性检出率分别为25.64%、12.82%、23.08%、3.85%;RUT检测胃溃疡、萎缩性胃炎、十二指肠溃疡、胃癌的Hp阳性检出率分别为26.92%、12.82%、20.51%、2.56%,无统计学差异(P>0.05)。结论:幽门螺杆菌测试仪碳14尿素呼气试验(C^(14)-UBT)在检测幽门螺杆菌感染中具有重要的临床价值,不仅准确性高,且操作简单,安全可行。
Objective:To discuss the clinical effects of C^(14)-UBT(14C-urea breath test)via testing instruments in helicobacter pylori(Hp)infections.Methods:78 patients with abdominal discomfort(2018.01-2019.01)were chosen as research samples.After admission,C^(14)-UBT and rapid urease test(RUT)were performed to compare its diagnosis sensitivity,accuracy and specificity;the detection rates of Hp among digestive tract disease patients were compared.Results:Hp positive rates based on C^(14)-UBT and RUT were 65.38%and 62.82%.A total of 50 positive Hp cases were discovered(64.10%);The diagnosis sensitivity and specificity based on C^(14)-UBT were 96.0%and 89.29%;Youden index was 0.852.The diagnosis sensitivity and specificity based on RUT were 88.0%and 82.14%;Youden index was 0.793;The differences were not statistically significant(P>0.05);Hp positive detection rates of C^(14)-UBT among gastric ulcer,atrophic gastritis,duodenal ulcer and gastric cancer patients were 25.64%,12.82%,23.08%and 3.85%;Hp positive detection rates of RUT among gastric ulcer,atrophic gastritis,duodenal ulcer and gastric cancer patients were 26.92%,12.82%,20.51%and 2.56%;The differences were not statistically significant(P>0.05).Conclusion:C^(14)-UBT can increase the diagnosis accuracy,simply the operation procedures and improve the detection safety for Hp infection patients.
