声带良性肿物的特征与嗓音障碍程度的相关性分析 被引量:1

摘要 目的探讨频闪喉镜下声带良性肿物的特征与嗓音障碍程度的相关性。方法选取2019年6月1日~2020年8月31日在中国科学技术大学附属第一医院就诊的231例声带良性肿物患者,将他们的性别、年龄、病程、频闪喉镜下声带良性肿物的类型、占位比例、侧别、声带运动情况进行回顾性分析,并对患者的客观性声学参数进行检测,K^(-)均值聚类分析后采用多因素Logistic回归模型,分析声带良性肿物特征与嗓音障碍程度的相关性。结果将声压级(sound pressure level,SPL)、基频(fundamental frequency,F0)、基频微扰(jitter)和振幅微扰(shimmer)运用K^(-)均值聚类分析后将231例患者分为轻症组(168人)和重症组(63人);喉镜下肿物占位比例与嗓音障碍程度呈正相关(P<0.05),占位比例越大,嗓音障碍程度越高;并且性别与嗓音障碍程度具有相关性(P<0.05),男性患者嗓音障碍程度高于女性患者;声带良性肿物的病变侧别、类型、患者的年龄、病程与嗓音障碍程度均不存在相关性(P>0.05)。结论K^(-)均值聚类分析可以统计嗓音障碍程度,频闪喉镜下声带良性肿物的占位比例与嗓音障碍程度具有相关性,频闪喉镜下声带良性肿物特征对嗓音状况的预测以及治疗方式的选择有一定意义。 Objective To investigate the correlation between the characteristics of vocal cord benign tumor and the degree of voice attenuation under stroboscopic laryngoscope.Methods A total of 231 patients were selected in the First Affiliated Hospital of China University of science and technology from June 1,2019 to August 31,2020.Their gender,age,course of disease,type of benign vocal cord tumor under stroboscopic laryngoscope,space occupying ratio,side,vocal cord movement were retrospectively analyzed.The objective acoustic parameters of the patients were detected,and the correlation between the characteristics of vocal cord benign tumor and the degree of voice attenuation was analyzed by using multivariate logistic regression model after K^(-)means cluster analysis.Results The sound pressure level(SPL),fundamental frequency(F0),jitter and shimmer are analyzed by K^(-)means clustering,231 patients were divided into mild group(168 cases)and severe group(63 cases).There was a positive correlation between the proportion of tumor and the degree of voice attenuation(P<0.05).And gender was correlated with the degree of voice attenuation(P<0.05),the degree of voice attenuation in male patients was higher than that in female patients.There was no correlation between the type of benign vocal cord tumor,side of lesion,age,course of disease and the degree of voice attenuation(P>0.05).K^(-)means clustering analysis can count the degree of voice attenuation.The proportion of benign vocal cord tumor under stroboscopic laryngoscope is related to the degree of voice attenuation.Conclusion K^(-)means clustering analysis can count the degree of voice attenuation.The proportion of benign vocal cord tumor under stroboscopic laryngoscope is related to the degree of voice attenuation.The characteristics of benign vocal cord tumor under stroboscopic laryngoscope have certain significance for predicting voice condition and the choice of treatment methods.
