个案管理模式在单侧唇裂综合治疗中的应用

Synergism of case management nursing model combined with comprehensive treatments for pediatric patients with unilateral cleft lip treated by surgery

目的评价个案管理模式在单侧唇裂患儿综合治疗中的术后辅助增效作用。方法62例非综合征型单侧唇裂患儿随机分为对照组和实验组,每组31例。对照组患儿实施常规管理模式,实验组患儿实施个案管理模式,比较分析不同护理管理模式对二组患儿术后唇部切口瘢痕形成以及患儿家长负面情绪(抑郁、焦虑、压力)的影响,评估其在此类患儿综合治疗中的术后辅助增效作用。结果相对于常规管理模式组,个案管理模式组中的VSS评分明显降低,促进了唇部切口的有效愈合,瘢痕增生减少,唇裂修复术的远期美观效果提升。同时,也能帮助患儿家长改善心理状态,缓解负面情绪,抑郁、焦虑和压力得分均明显降低。结论个案管理护理模式应用于单侧唇裂患儿的综合治疗,能够有效增进术后康复效果。

Objective To evaluate the auxiliary postoperative synergistic effect of case management nursing model combined with comprehensive treatments on pediatric patients with unilateral cleft lip treated by surgery.Methods Included in this study were 62 pediatric cases with non-syndromic unilateral cleft lip,randomly divided into control group(CG)and experimental group(EG),with 31 in each.Routine management nursing model was given to cases in CG,and case management nursing model was performed in EG,both combined with comprehensive treatments for these cases respectively.Then,comparatively observed and analyzed between these two groups were the effects of different management nursing models on their postoperative scar formation in surgical incision of lip among these children and on three negative emotional states,i.e.depression,anxiety and stress,among the parents of these pediatric patients to observe the auxiliary synergistic effect of different nursing models carried out during the process of comprehensive treatments of the cases.Results Compared with the results of CG,the auxiliary synergistic effects of case management nursing model carried out in EG were shown with significantly much better auxiliary synergistic effect following operation,with obviously decreased Vancouver scar scale(VSS)scores,much better healing outcome in lip incision,markedly reduced scar formation and very satisfied cosmetic effects.Meanwhile,the emotional states were significantly improved among the parents of these pediatric patients,with their negative emotional states relieved largely,the severities of depression,anxiety and stress decreased significantly.Conclusion Case management nursing model can give rise a significantly effective auxiliary synergistic effect on postoperative pediatric patients with unilateral cleft lip when combined in their comprehensive treatments.
