DKK2基因启动子甲基化及高危HPV感染在宫颈癌中的临床意义

摘要 目的探讨DKK2基因启动子甲基化及高危型人乳头瘤病毒(HR-HPV)感染在宫颈癌中的临床意义。方法收集2016年5月至2017年11月该院确诊的79例宫颈癌患者(宫颈癌组)及63例宫颈癌前病变患者(癌前病变组)组织标本,其中高级别鳞状上皮内病变(HSIL)38例,低级别鳞状上皮内病变(LSIL)25例。同时选取29例子宫平滑肌瘤行子宫全切的患者作为对照组。比较3组患者DKK2基因启动子甲基化及HR-HPV感染情况。结果癌前病变组中HSIL患者DKK2基因启动子甲基化率与宫颈癌组、对照组患者及癌前病变组中LSIL患者比较,差异有统计学意义(P<0.05)。3组患者HR-HPV感染率及感染者DKK2基因启动子甲基化率比较,差异有统计学意义(P<0.05)。宫颈癌组中,HPV16/18阳性患者DKK2基因启动子甲基化率高于非HPV16/18阳性患者,差异有统计学意义(P<0.05)。癌前病变组中,HPV16/18阳性患者DKK2基因启动子甲基化率高于非HPV16/18阳性患者,但差异无统计学意义(P>0.05)。结论DKK2常因其启动子高甲基化而失活,与宫颈癌的发生发展相关,可能是一种潜在的宫颈癌分子标志物。 Objective To investigate the clinical significance of DKK2 gene promoter methylation and high-risk human papillomavirus(HR-HPV)infection in cervical cancer.Methods Tissue samples were col-lected from 79 patients with cervical cancer(the cervical cancer group)and 63 patients with cervical precancer-ous lesions(the precancerous lesions group)diagnosed in the hospital from May 2016 to November 2017,in-cluding 38 cases of high-grade squamous intraepithelial lesions(HSIL)and 25 cases of low-grade squamous in-traepithelial lesions(LSIL).At the same time,29 patients with uterine leiomyoma undergoing total hysterecto-my were selected as the control group.The methylation of DKK2 gene promoter methylation and HR-HPV infection were compared among the three groups.Results The DKK2 gene promoter methylation rate of HSIL patients in the precancerous lesions group was significantly different from that of patients in the cervical cancer group,the control group and LSIL patients in the precancerous lesion group(P<0.05).There were significant differences in HR-HPV infection rate and DKK2 gene promoter methylation rate among the three groups(P<0.05).In the cervical cancer group,the methylation rate of DKK2 gene promoter in HPV16/18 positive patients was higher than that in non HPV16/18 positive patients,and the difference was statistically significant(P<0.05).In the precancerous lesions group,the methylation rate of DKK2 gene promoter in HPV16/18 positive patients was higher than that in non HPV16/18 positive patients,and the difference was not statistically sig-nificant(P>0.05).Conclusion DKK2 is often inactivated by hypermethylation of its promoter,which is relat-ed to the occurrence and development of cervical cancer,and may be a potential molecular marker of cervical cancer.
