摘要
目的:探讨在妊娠期高血压疾病中应用无创性血流动力学监测系统评估子痫前期患者治疗前后心功能的价值。方法:选取2019年4月-2020年10月常州市妇幼保健院产科收治的182例妊娠期高血压患者,按妊娠期高血压疾病诊治指南(2020版)进行分组,子痫前期纳入98例记为研究组1,重度子痫前期纳入84例记为研究组2。对所统计的心血管血流参数无损伤检测资料展开分析。对两组血压变化、相关指标以及无创性血流动力学展开评测对比。结果:研究组1患者24 h、日间及夜间的收缩压和舒张压低于研究组2,差异有统计学意义(P<0.05)。研究组1患者平均动脉压、胸液水平、外周血管阻力低于研究组2,研究组1患者心脏指数和心输出量高于研究组2,差异有统计学意义(P<0.05)。研究组1患者经对血流动力学分型展开评估工作,高排低阻型、高排高阻型其所占比值高于研究组2,研究组1低排高阻型所占比例低于研究组2,差异有统计学意义(P<0.05)。结论:无创性血流动力学监测系统对于子痫前期预后判断具有良好的指导作用,应用价值显著。
Objective:To explore the value of noninvasive hemodynamic monitoring system in the evaluation of cardiac function in gestational hypertension patients with preeclampsia before and after treatment.Methods:A total of 182 patients with gestational hypertension in Changzhou Maternal and Child Health Care Hospital from April 2019 to October 2020 were selected.According to the guidelines for the diagnosis and treatment of pregnancy induced hypertension(2020 Edition),98 cases of preeclampsia were recorded as study group 1 and 84 cases of severe preeclampsia were recorded as study group 2.The statistical data of noninvasive detection of cardiovascular blood flow parameters were analyzed.The changes of blood pressure,related indexes and noninvasive hemodynamics were evaluated and compared between the two groups.Results:The systolic blood pressure and diastolic blood pressure of patients in study group 1 at 24 h,daytime and night were lower than those in study group 2,and the difference was statistically significant(P<0.05).The mean arterial pressure,pleural fluid level and peripheral vascular resistance of patients in study group 1 were lower than those in study group 2,and the cardiac index and cardiac output of patients in study group 1 were higher than those in study group 2,and the differences were statistically significant(P<0.05).The hemodynamic typing of patients in study group 1 was evaluated.The proportions of high-output,low-resistance and high-output,high-resistance types were higher than those in study group 2,and the proportion of low-output,high-resistance types in study group 1 was lower than that in the study group 2,and the difference was statistically significant(P<0.05).Conclusion:Noninvasive hemodynamic monitoring system plays a good guiding role in judging the prognosis of preeclampsia and has a significant application value.
