柴附六君汤治疗桥本氏甲状腺炎甲减肝郁脾虚证的临床效果

Clinical Observation of Chaifu Liujun Decoction in the Treatment of Hypothyroidism of Hashimoto's Thyroiditis with Liver Depression and Spleen Deficiency Syndrome

目的观察桥本氏甲状腺炎甲减肝郁脾虚证治疗中柴附六君汤的临床应用效果。方法回顾性选取2020年2月至2021年2月本院桥本氏甲状腺炎甲减肝郁脾虚证患者60例,依据用药方法分为左甲状腺素钠片单独治疗组(单独用药组)、左甲状腺素钠片联合柴附六君汤治疗组(联合用药组)两组,各30例,统计分析两组中医症候积分、甲状腺自身抗体滴度、甲状腺功能、临床疗效。结果用药前,两组患者的中医症候积分、TGAb、TPOAb、TSH、TT3、FT3、TT4、FT4水平之间的差异均不显著(P>0.05),用药后,联合用药组患者的中医症候积分、TGAb、TPOAb、TSH水平均低于单独用药组(P<0.05),TT3、FT3、TT4、FT4水平均高于单独用药组(P<0.05);两组患者用药后的中医症候积分、TGAb、TPOAb、TSH水平均低于用药前(P<0.05),TT3、FT3、TT4、FT4水平均高于用药前(P<0.05)。联合用药组患者的总有效率96.67%(29/30)高于单独用药组60.00%(18/30)(χ^(2)=11.882,P<0.05)。结论桥本氏甲状腺炎甲减肝郁脾虚证治疗中柴附六君汤的临床应用效果好。

Objective To observe the clinical effect of Chaifuliujun Decoction in the treatment of hypothyroidism of Hashimoto’s thyroiditis.Methods A total of 60 patients with hypothyroidism, liver depression and spleen deficiency syndrome of Hashimoto’s thyroiditis in our hospital from February 2020 to February 2021 were retrospectively selected. According to the medication method, they were divided into two groups: left thyroxine sodium tablet alone treatment group(alone treatment group) and left thyroxine sodium tablet combined with Chaifuliujun decoction treatment group(combined treatment group), with 30 cases in each group. The TCM symptom scores of the two groups were statistically analyzed Thyroid autoantibody titer, thyroid function and clinical efficacy.Results Before treatment, there was no significant difference in TCM symptom score, TGAb, TPOAb, TSH, TT3, FT3,TT4 and FT4 levels between the two groups(P>0.05).After treatment, the TCM symptom score, TGAb, TPOAb and TSH levels in the combined treatment group were lower than those in the single treatment group(P<0.05),and the levels of TT3,FT3, TT4 and FT4 were higher than those in the single treatment group(P<0.05);The TCM symptom score, TGAb, TPOAb and TSH levels of the two groups after treatment were lower than those before treatment(P<0.05),and the levels of TT3, FT3, TT4 and FT4 were higher than those before treatment(P<0.05).The total effective rate of 96.67%(29/30)in the combination group was higher than 60.00%(18/30)in the single drug group(χ^(2)=11.882,P<0.05).Conclusion Hashimoto’s thyroiditis has a good clinical effect in the treatment of liver depression and spleen deficiency syndrome of hypothyroidism.
