超声标记肾脏体表投影测量肾脏移动度

摘要 目的观察基于超声标记肾脏体表投影测量肾脏移动度新方法的可行性。方法纳入338名参加2021年度空军飞行学员医学选拔定选阶段的健康应征学生,以超声定位仰卧及直立位下双肾上、下极、肋弓下缘及髂前上棘体表投影,测量双肾长度、肾脏直接移动度及间接移动度。以组内相关系数(ICC)评价体表投影法与超声常规法测量肾脏长度的一致性,以及体表投影法所测双肾上极相对于肋弓下缘的间接移动度与下极相对于肋弓下缘的间接移动度的一致性,比较肾脏上/下极间接移动度与直接移动度的差异。结果2种方法测量仰卧、直立位下肾脏长度的一致性较好(ICC均>0.70,P均<0.001)。体表投影法所测左、右肾脏间接移动度均大于直接移动度(P均<0.05)。体表投影法所测双肾上极相对于肋弓下缘的间接移动度与下极相对于肋弓下缘的间接移动度测值一致性最佳,ICC分别为0.911、0.923(P均<0.001)。结论超声标记肾脏体表投影测量肾脏移动度方法可行;以肾脏下极与肋弓下缘作为标准定位点准确、合理。 Objective To observe the feasibility of measuring renal mobility based on ultrasound labeled renal surface projection.Methods Totally 338 healthy candidates who participated in the medical selection stage of air force flight cadets in 2021 were enrolled.The body surface projections of the upper and lower poles of the kidneys,the lower edge of the costal arch and the anterior superior iliac spine in the supine and upright position were located with ultrasound.The length of the kidneys,the direct and indirect movements of the kidneys were measured.Intra-class correlation coefficient(ICC)was used to evaluate the consistency of kidney length measured with body surface projection and conventional ultrasound,also of indirect mobility of the upper pole of both kidneys relative to the lower edge of the costal arch the lower pole relative to the lower edge of the costal arch measured with body surface projection.The indirect and direct mobility of the upper/lower pole of the kidneys were compared.Results There were good agreements between body surface projection and conventional ultrasound method in measuring the lengths of kidneys at supine and upright position(all ICC>0.70,all P<0.001).The indirect mobility of the left and right kidneys measured with body surface projection were longer than the direct mobility(all P<0.05).The indirect mobility of the upper pole of both kidneys relative to the lower edge of the costal arch measured with body surface projection were the best consistent with the indirect mobility of the lower pole relative to the lower edge of the costal arch,with ICC of 0.923 and 0.911,respectively(both P<0.001).Conclusion Measuring renal mobility based on ultrasound labeled renal surface projection was feasible.Taken the lower pole of the kidney and the lower edge of the costal as the standard positioning points,measuring the indirect mobility of kidneys was accurate and reasonable.
