Gal-3在射血分数保留的心衰患者血清中的表达及其诊断价值

Expressions of Galectin-3 in Serum of Patients with Heart Failure with Preserved Ejection Fraction and its Diagnostic Value

目的研究半乳糖凝集素3(Gal-3)在射血分数保留的心力衰竭(HFpEF)的表达及其诊断价值。方法选取2021年4月至5月于我院就诊的HFpEF患者84例(HFpEF组),按照美国纽约心脏病学会(NYHA)心功能分级分组:心功能Ⅱ级组40例,心功能Ⅲ级组20例,心功能Ⅳ级组24例;选取同期健康体检者24例作为对照组。检测其血清中Gal-3及脑钠肽(BNP)的水平。结果各亚组的Gal-3及BNP水平与对照组均有明显差异(P<0.05),且HFpEF组的各亚组的Gal-3均值及BNP均值高于对照组,NYHAⅡ、Ⅲ及Ⅳ级两两之间比较,差异有统计学意义(P<0.05)。相关性分析中lnGal-3与NYHA分级(r=0.943,P<0.001)、lnBNP(r=0.922,P<0.001)、左室前后径(LVD)(r=0.282,P=0.039)及左房前后径(LAD)(r=0.782,P<0.001)呈显著正相关,与射血分数(LVEF)呈显著负相关(r=-0.755,P<0.001)。结论HFpEF患者血清中Gal-3水平在一定程度上可反映HFpEF的严重程度,对HFpEF早期的诊断有一定参考价值。

Objective To study expression and diagnostic value of Galectin-3(Gal-3) in heart failure with preserved ejection fraction(HFpEF).Methods From April to May 2021,84 HFpEF patients(HFpEF group) who were treated in our hospital were selected.According to New York Heart Association(NYHA) cardiac function classification, there were 4 groups: NYHA Ⅱ group(n=40),NYHA Ⅲ group(n=20),NYHA IV group(n=24).24 healthy subjects were selected as control group during the same period.Serum Gal-3 and brain natriuretic peptide(BNP) were detected.Results Gal-3 and BNP in each subgroup were significantly different from those in control group(P<0.05),and mean values of Gal-3 and BNP in each subgroup in HFpEF group were higher than those in control group.There was statistical significance in comparison of NYHA Ⅱ,Ⅲ and IV groups(P<0.05).In correlation analysis, lnGal-3 and NYHA class(r=0.943,P<0.001),lnBNP(r=0.922,P<0.001),left ventricular anteroposterior diameter(LVD)(r=0.282,P=0.039),left ventricular anterior and posterior atrial diameter(LAD)(r=0.782,P<0.001) were significantly positively correlated, and significantly negatively correlated with ejection fraction(LVEF)(r=-0.778,P<0.001).Conclusion Serum Gal-3 of HFpEF patients could reflect HFpEF severity to a certain extent, and would have a certain reference value to early diagnose HFpEF.
