乐山市7~9岁儿童龋齿患病率调查及与体质量的相关性研究 被引量:4

摘要 目的调查乐山市7~9岁儿童龋齿患病率,并探讨儿童龋齿与体质量的相关性。方法根据2003年中国学组推荐的BMI分界值,将336例7~9岁的小学生分为体质量不足组、体质量正常组、体质量超重组和肥胖组,由医师对儿童进行全口龋病检测。采用Logistic回归法分析BMI对儿童龋齿患病率的影响。应用限制性三次样条回归法分析年龄的体块指数Z评分(BAZ)与乳牙龋病的关系。结果①随着年龄增长,男童和女童的BMI都逐渐升高。7岁组、8岁组、9岁组男童龋齿患病率分别为31.1%(19/61)、36.7%(18/49)、31.8%(21/66),女童患病率分别为27.3%(15/55)、31.2%(15/48)、15.8%(9/51),在7岁组和8岁组,男童与女童的龋齿患病率差异无统计学意义(P=0.647;P=0.569),但在9岁组,男童的龋齿患病率明显高于女童(P=0.039)。②正常体质量、体质量不足、超重和肥胖的男童分别为55.1%、7.4%、24.4%、13.1%;女童分别为56.9%、6.9%、21.9%、14.4%。在男童和女童中,随着BMI的增加,儿童的龋齿患病率逐渐升高(P<0.05)。调整年龄和性别因素后,超重和肥胖儿童乳牙龋病患病率分别降低29.6%和35.5%。结论7~9岁儿童儿童龋病与BMI呈负相关,超重和肥胖儿童发生乳牙龋病的风险明显较高。 Objective To investigate the correlation between body weight and incidence rate of dental caries of 7~9 years old children in Leshan City.Methods 336 pupils aged 7~9 years were divided into underweight group,normal weight group,overweight group and obesity group,according to the BMI cut-off value recommended by the 2003 Chinese academic Group.Logistic regression was used to calculate the influence of age and BMI on the incidence rate of dental caries in children.The relationship between Baz and deciduous dental caries was analyzed by restricted cubic spline regression.Results①The BMI of boys and girls increased gradually with age.The prevalence rates of dental caries in boys in 7-year-old group,8-year-old group and 9-year-old group were 31.1%,36.7%and 31.8%respectively,and the prevalence rates of dental caries in girls were 27.3%,31.2%and 15.8%respectively.There was no significant difference between boys and girls in 7-year-old group and 8-year-old group(P=0.647);P=0.569),but in the 9-year-old group,the prevalence of dental caries in boys was significantly higher than that in girls(P=0.039).②Normal weight,underweight,overweight and obese boys were 55.1%,7.4%,24.4%and 13.1%respectively,while girls were 56.9%,6.9%,21.9%and 14.4%respectively.In boys and girls,the prevalence of dental caries increased gradually with the increase of BMI(P<0.05).The prevalence of primary dental caries in overweight and obese children decreased by 29.6%and 35.5%respectively.Conclusion The prevalence of primary dental caries in children aged 7~9 years decreases with age.At the same time,there is a negative correlation between childhood caries and BMI.Overweight and obese children had a significantly higher risk of primary dental caries.
