盐水稀酸稀碱三步法在白带检查中的应用价值

Application value of saline dilute acid dilute alkali three-step method in leucorrhea examination

目的 探究盐水稀酸稀碱三步法在白带检查中的应用价值。方法 选取2020年1月1日至2020年12月31日在本院妇科门诊就诊的1 000例患者作为研究对象。采集患者的白带标本并进行生理盐水涂片法、革兰氏染色法、盐水稀酸稀碱三步法检验。通过妇科相关检查联合经阴道超声检查得出临床最终确诊结果(金标准),比较三种检验方法对阴道清洁度的判定结果、诊断效能及对病原体的检出情况。结果 生理盐水涂片法、革兰氏染色法、盐水稀酸稀碱三步法检验阴道清洁度异常情况的符合率分别为56.59%(472/834)、77.34%(645/834)、98.32%(820/834)。盐水稀酸稀碱三步法检验阴道清洁度的符合率、异常率均高于生理盐水涂片法、革兰氏染色法,差异具有统计学意义(P<0.05)。盐水稀酸稀碱三步法检验阴道清洁度的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于生理盐水涂片法、革兰氏染色法,差异具有统计学意义(P<0.05)。盐水稀酸稀碱三步法检出细菌感染、真菌感染、滴虫感染的符合率均高于生理盐水涂片法、革兰氏染色法,差异具有统计学意义(P<0.05)。结论 盐水稀酸稀碱三步法在白带检查中的应用价值显著,可有效检出阴道清洁度及相关病原体,具有较高的诊断效能,值得临床推广和应用。

Objective To explore the application value of saline dilute acid dilute alkali three-step method in leucorrhea examination.Methods From January 1,2020 to December 31,2020,1000 patients in the gynecological outpatient department of our hospital were selected as the research objects.The leucorrhea specimens of the patients were collected and tested by normal saline smear method,Gram staining method and saline dilute acid dilute alkali three-step method.The final clinical diagnosis result(gold standard)was obtained through gynecological related examinations combined with transvaginal ultrasonography,and the results of vaginal cleanliness determination,diagnostic efficiency and pathogen detection of the three test methods were compared.Results The coincidence rates of abnormal vaginal cleanliness of normal saline smear method,Gram staining method and saline dilute acid dilute alkali three-step method were 56.59%(472/834),77.34%(645/834),98.32%(820/834)respectively.The coincidence rate and abnormality rate of vaginal cleanliness test by saline dilute acid dilute alkali three-step method were higher than those by normal saline smear method and Gram staining method,and the differences were statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of saline dilute acid dilute alkali three-step method for vaginal cleanliness were higher than those of normal saline smear method and Gram staining method,and the differences were statistically significant(P<0.05).The coincidence rates of bacterial infection,fungal infection and trichomonad infection detected by saline dilute acid dilute alkali three-step method were higher than those by normal saline smear method and Gram staining method,and the differences were statistically significant(P<0.05).Conclusion The saline dilute acid dilute alkali three-step method in leucorrhea examination has significant application value,it can effectively detect vaginal cleanliness and related pathogens,and has higher diagnostic efficiency,which is worthy of clinical promotion and application.
