PDCA循环在静脉药物配置中心曲妥珠单抗余药管理中的应用效果

Application effect of PDCA cycle on the management of residual trastuzumab in pharmacy intravenous admixture services

目的 分析PDCA循环在静脉药物配置中心(PIVAS)曲妥珠单抗余药管理中的应用效果,以节约医疗资源。方法选取我院2018年9月至2019年5月收治的30例乳腺癌患者为对照组,对其实施常规余药管理模式;选取我院2020年6月至2020年12月收治的30例乳腺癌患者为研究组,对其实施PDCA循环管理模式。比较两组患者使用曲妥珠单抗的相关数据(实际用药总量、余药失效总量、失效总金额、余药失效率、次均用量、次均金额)、配置时间、管理时间。结果 经过PDCA循环管理模式对工作流程的持续优化改进,完善了曲妥珠单抗余药的贮存、记录与查询,研究组的实际用药总量、余药失效总量、失效总金额、余药失效率均小于对照组;研究组的次均用量、次均金额均少于对照组,差异具有统计学意义(P<0.05)。研究组的配置时间、管理时间均短于对照组,差异具有统计学意义(P<0.05)。结论将PDCA循环管理模式应用于曲妥珠单抗余药的管理中,并制定规范的登记配置存放流程,可保证药物疗效,提高工作效率,保障药品质量与安全,减少患者住院金额,提高药学服务质量。

Objective To analyze the application effect of PDCA cycle on the management of residual trastuzumab in pharmacy intravenous admixture services(PIVAS),in order to save medical resources.Methods A total of 30 patients with breast cancer admitted in our hospital from September 2018 to May 2019 were selected as control group,and implemented the routine residual drug management mode;30 patients with breast cancer admitted in our hospital from June 2020 to December 2020 were selected as study group,and implemented the PDCA cycle management mode.The relevant data(actual total amount of medication,total amount of residual drug lose efficacy,total amount of lose efficacy,lose efficacy rate of residual drug,average dosage of per time,average amount of per time),configuration time and management time of trastuzumab were compared between the two groups.Results Through the continuous optimization and improvement of the work process through the PDCA cycle management mode,the storage,recording and query of residual trastuzumab have been improved,and the actual total amount of medication,total amount of residual drug lose efficacy,total amount of lose efficacy and lose efficacy rate of residual drug in the study group were less than those in the control group;the average dosage of per time and average amount of per time in the study group were less than those in the control group,and the differences were statistically significant(P<0.05).The configuration time and management time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Applying the PDCA cycle management mode to the management of residual trastuzumab and formulating a standardized registration,configuration and storage process can ensure the efficacy of drugs,improve work efficiency,ensure the quality and safety of drugs,reduce the amount of hospitalized patients,and improve the quality of pharmaceutical care.
