利伐沙班对老年稳定性冠心病伴心房颤动患者的影响

Effect of Rivaroxaban on Elderly Patients with Stable Coronary Heart Disease Complicated with Atrial Fibrillation

目的:分析利伐沙班对老年稳定性冠心病(CHD)伴心房颤动(AF)患者左心室功能、心肌缺血标志物的影响。方法:选取安阳市第三人民医院2019年6月至2020年8月收治的74例老年稳定性CHD伴AF患者,随机法分为对照组和观察组,各37例。对照组采取常规治疗,观察组在对照组基础上加用利伐沙班治疗,比较两组患者治疗效果。结果:观察组患者治疗后舒张末期左心室后壁厚度(LVPWD)、舒张晚期峰值速度(A)、心肌缺血标志物水平、发作持续时间、发作频率、发作时ST段压低程度低于对照组,舒张早期最大峰值速度(E)高于对照组,差异具有统计学意义(P<0.05);观察组患者不良反应发生率(5.41%)低于对照组(21.62%),差异具有统计学意义(P<0.05)。结论:对老年稳定性CHD伴AF患者采取利伐沙班治疗效果较好,可有效缩短其发作持续时间,改善其心肌缺血标志物水平。

Objective To investigate the effects of rivaroxaban on left ventricular function and myocardial ischemia markers in elderly patients with stable coronary heart disease(CHD)complicated with atrial fibrillation(AF).Methods 74 elderly patients with stable CHD complicated with AF admitted to Anyang Third People's Hospital from June 2019 to August 2020 were randomly divided into a control group and an observation group,with 37 cases in each group.The control group received conventional treatment,and the observation group was treated with rivaroxaban on the basis of the control group,and the treatment effect of the two groups were compared.Results After treatment,the end-diastolic left ventricular posterior wall depth(LVPWD),late diastolic peak velocity(A),the levels of myocardial ischemia markers,duration of attack,attack frequency,ST segment depression at the attack in the observation group were lower than those of the control group,early diastolic peak velocity(E)was higher than that of the control group,the differences were statistical significance(P<0.05).The incidence of adverse reactions in the observation group was 5.41%lower than 21.62%in the control group,and the difference was statistically significant(P<0.05).Conclusion Rivaroxaban has a better effect on elderly patients with stable CHD with AF,which can effectively shorten the duration of attack and improve the level of myocardial ischemia markers.
