糖尿病酮症酸中毒合并鼻-眼-脑曲霉感染1例

A case of diabetic ketoacidosis complicated with acute explosive rhino-orbital-cerebral Aspergillus infection

24岁男性患者,头痛、流涕3天,腹痛1天入院。期间症状急性加重,出现左侧肢体偏瘫、右眼盲等症状。经查体、影像学、病理学等明确诊断为急性爆发性鼻-眼-脑曲霉感染。经盐酸米诺环素联合卡泊芬净治疗后症状得到控制,后择期进行了开颅手术治疗。发病后1年随访,患者偏瘫卧床,右眼视力有光感,整体病情稳定。

A 24-year-old male patient was admitted with headache,runny nose for 3 days and abdominal pain for 1 day.During this period,the symptoms were acutely aggravated,including hemiplegia of the left limb and blindness of the right eye.After physical examination,imaging and pathology,it was definitely diagnosed as acute explosive rhino-orbital-cerebral Aspergillus infection.After treatment with minocycline hydrochloride combined with caspofungin,the symptoms were controlled and craniotomy was performed at a later time.One year follow-up after the onset of the disease,the patient was lying in bed with hemiplegia,and the right eye vision had light perception,and the overall condition was stable.
