宿迁地区健康成人血清肌酐参考区间的建立

Establishment of creatinine reference interval for healthy population in Suqian area

目的检测宿迁市健康成人的肌酐水平,建立适合本实验室的肌酐参考区间,并与行业标准及厂商提供的参考区间进行比较。方法选取2020年1—12月于本院进行体检的28561名健康人群肌酐数据,男13765名,女14796名,按性别和年龄为男性20~59岁组(n=10358),男性60~79岁组(n=3047);女性20~59岁组(n=12231),女性60~79岁组(n=2565)。比较不同性别、年龄血清肌酐;分析不同性别、不同年龄组血清肌酐的自建参考区间及自建参考区间与行业标准及厂商的相对偏差。结果男性血清肌酐水平高于女性,差异有统计学意义(P<0.05)。男性20~59岁组血清肌酐水平低于男性60~79岁组;女性20~59岁组低于女性60~79岁组,差异有统计学意义(P<0.05)。由于血清肌酐检测结果呈偏态分布,本研究建立的血清肌酐参考范围如下:男性20~59岁组血清肌酐范围52~92μmol/L,男性60~79岁组为51~101μmol/L,女性20~59岁组为39~67μmol/L,女性60~79岁为38~73μmol/L。自建参考区间与行业标准相对偏差d值小于RCV,且与厂商相对偏差d值大于RCV(RCV=11.09%)。结论根据不同年龄和性别初步建立合适宿迁地区健康成人的肌酐参考区间,不盲目应用厂商参考区间,为临床的诊断、治疗及预后提供更具价值的依据。

Objective To detect the creatinine level of healthy adults in Suqian City,establish a creatinine reference interval suitable for our laboratory,and compare it with the industry standard and the reference interval provided by the manufacturer.Methods The creatinine data of 28561 healthy people who underwent physical examinations from January to December 2020 were selected.there were 13765 males and 14796 females.they were divided into male 20-59 years old group(n=10358),male 60-79 year old group(n=3047);female 20-59 years old group(n=12231),female 60-79 years old group(n=2565).According to gender and age serum creatinine by gender and age were compared;the self-established reference interval of serum creatinine in different gender and age groups and the relative deviation of the self-established reference interval from industry standards and manufacturers were analyzed.Results The serum creatinine level in male was higher than that in female,and the difference was statistically significant(P<0.05).The serum creatinine level in the male 20-59 years old group was lower than that in the male 60-79 year old group;the female 20-59 year old group was lower than that in the female 60-79 year old group,and the difference was statistically significant(P<0.05).Due to the skewed distribution of serum creatinine test results,the reference range of serum creatinine established in this study is as follows:the range of serum creatinine in male 20-59 years old group was 52-92μmol/L,the male 60-79 year old group was 51-101μmol/L,the female 20-59 year old group was 39-67μmol/L,and the female 60-79 year old group was 38-73μmol/L.The relative deviation d between the self-built reference interval and the industry standard was smaller than the RCV,and the relative deviation d from the manufacturer was larger than the RCV(RCV=11.09%).Conclusion According to different ages and genders,the creatinine reference interval suitable for healthy adults in Suqian area is initially established,and the manufacturer's reference interval is not blindly applied to provide more valuable evidence for clinical diagnosis,treatment and prognosis.
