纤支镜吸痰联合抗感染药物治疗重症肺部感染患者的临床疗效

Clinical efficacy of bronchoscopy for sputum aspiration combined with anti-infective drugs in the treatment of patients with severe pulmonary infection

目的探讨纤维支气管镜吸痰联合抗感染药物治疗重症肺部感染患者的临床疗效。方法选取2017年6月至2019年1月本院收治的50例重症肺部感染患者作为研究对象,采用抛硬币法分为两组,各25例。对照组行纤维支气管镜吸痰治疗,观察组在对照组基础上行抗感染药物治疗。比较两组临床疗效、肺功能[第1秒用力呼气容积(FEV1)、最大通气量(MMV)、最大呼气中期流量(MMEF)、功能残气量(FRC)、肺总量(TLC)]及症状缓解时间。结果观察组治疗总有效率略高于对照组,但差异无统计学意义。治疗后,观察组FEV1、MMV、MMEF、FRC及TLC水平均高于对照组,差异有统计学意义(P<0.05)。观察组恢复体温正常时间、痰量减少时间、啰音消失时间及住院时间均短于对照组,差异有统计学意义(P<0.05)。结论在抗感染药物治疗的基础上联合纤支镜吸痰治疗重症肺部感染,可快速恢复患者的肺功能,缓解临床症状,提高机体恢复效率,值得临床推广应用。

Objective To investigate the clinical efficacy of bronchoscopy for sputum aspiration combined with anti-infective drugs in the treatment of patients with severe pulmonary infection.Methods 50 patients with severe pulmonary infection admitted to our hospital from June 2017 to January 2019 were selected as the research subjects,and they were divided into two groups by coin toss method,with 25 cases in each group.The control group was treated with bronchoscopy for sputum suction,and the observation group was treated with anti-infective drugs on the basis of the control group.The clinical efficacy,pulmonary function(forced expiratory volume in one second[FEV1],maximum ventilation volume[MMV],midterm flow rate[MMEF],functional residual capacity[FRC],and total lung capacity[TLC])and symptom relief time were compared between the two groups.Results The total effective rate of treatment in the observation group was slightly higher than that in the control group,but the difference was not statistically significant.After treatment,the levels of FEV1,MMV,MMEF,FRC and TLC in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The time to return to normal body temperature,time for sputum volume reduction,disappearance time of rales and hospital stay in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).Conclusion On the basis of anti-infective drug treatment,combined with bronchoscopy for sputum suction in the treatment of severe pulmonary infection,can quickly restore the lung function of patients,relieve clinical symptoms,and improve the recovery efficiency of the body,which is worthy of clinical application.
