摘要
目的:探讨2D-STE对CRF患者心血管疾病的诊断价值。方法:回顾性分析30例CRF患者和30名健康志愿者。记录LA和LV大小、LVEF、E/A值、MV口流速、室壁厚度、PLS-En、PLS-M、PLS-Ep、PLS-G和左室GLS;ROC分析诊断价值。结果:CRF组MV口流速、LV大小明显更高,E/A比值明显更低(P<0.05),AUC为0.830、0.643、0.517;CRF组PLS-En、PLS-M、PLS-Ep、PLS-G和左室GLS绝对值明显降低(P<0.05),AUC为0.731、0.766、0.797、0.726、0.721,联合检测为0.891;CRF组CS和RS明显低于对照组(P<0.05),AUC为0.610、0.698,联合检测为0.711;长、短轴各水平切面各节段平均应变值诊断的AUC为0.938。结论:2D-STE对CRF患者心血管疾病早期诊断价值较高。
Objective:To explore the diagnostic value of two-dimensional speckle tracking echocardiography(2D-STE)for cardiovascular diseases in patients with chronic renal failure(CRF).Methods:A total of 30 patients with CRF and 30 healthy volunteers were retrospectively analyzed.The size of left atrium(LA)and left ventricle(LV),left ventricular ejection fraction(LVEF),E/A ratio,mitral valve(MV)flow velocity,ventricular wall thickness,PLS-En,PLS-M,PLS-Ep,PLS-G and global longitudinal strain(GLS)of left ventricle were recorded.And their diagnostic value was analyzed by ROC curves.Results:Compared with control group,MV flow velocity and LV size were significantly higher,and E/A ratio was significantly lower in CRF group(P<0.05).AUC values were 0.830,0.643 and 0.517,respectively.The absolute values of PLS-En,PLS-M,PLS-Ep,PLS-G and left ventricular GLS were significantly decreased in CRF group(P<0.05),and AUC values were 0.731,0.766,0.797,0.726 and 0.721,respectively.AUC of combined detection was 0.891.CS and RS in CRF group were significantly lower than those in control group(P<0.05),AUC values of CS,RS and combined detection were 0.610,0.698 and 0.711,respectively.The diagnostic AUC value of mean strain values in long axis and short axis was 0.938.Conclusion:2D-STE has good early diagnostic value for cardiovascular diseases in CRF patients.
