摘要
目的探究80 kV扫描结合人工智能(Artificial Intelligence,AI)优化技术在体重超重患者冠状动脉CT血管成像(Coronary Computed Tomography Angiography,CCTA)的临床应用。方法本前瞻性研究将2020年9月至2021年3月我院60例体重超重患者(BMI≥24 kg/m2)随机分为A、B两组,每组30例。A组采用120 kV扫描、ClearView+迭代技术。B组采用80 kV扫描,进一步将B组分为B1组和B2组,B1组采用ClearView+迭代技术,B2组采用AI优化技术。比较图像的主观评分、冠状动脉分支节段的CT值、噪声、信噪比(Signal to Noise Ratio,SNR)、对比噪声比(Contrast to Noise Ratio,CNR),以及辐射剂量。结果主观评价A、B2组均显著高于B1组(P<0.001),A、B2组无显著差异(P>0.05)。B组的CT值显著高于A组。B2组冠状动脉分支的CNR均显著高于A、B1组(P<0.05)。B组辐射指标均显著低于A组(P<0.001),且有效剂量(Effective Dose,ED)减低达75.18%。结论80 kV扫描结合AI优化技术在体重超重患者CCTA检查的图像质量与120 kV标准扫描相一致,且辐射剂量更低,可在临床推广应用。
Objective To explore the clinical application of 80 kV scanning combined with artificial intelligence(AI)optimization technique in coronary computed tomography angiography(CCTA)in overweight patients.Methods In this prospective study,a total of 60 overweight patients(BMI≥24 kg/m2)in our hospital from September 2020 to March 2021were randomly divided into group A and group B,with 30 patients in each group.Group A used 120 kV scanning and ClearView+iterative technique.80 kV scanning was used in group B,and group B was divided in to group B1 and group B2.Group B1 used ClearView+iterative technique,and group B2 used AI optimization technique.The subjective evaluation of images,CT values,noise,signal to noise ratio(SNR),contrast to noise ratio(CNR)of coronary branch segments,and radiation dose were compared.Results The subjective evaluations were significantly higher in both groups A and B2 than in group B1(P<0.001),and there was no significant difference between group A and group B2(P>0.05).The CT values of group B were significantly higher than those of group A.The CNR of coronary artery branches in group B2 were significantly higher than those in group A and group B1(P<0.05).The radiation indexes in group B were all significantly lower than in group A(P<0.001),and the effective dose(ED)was reduced by 75.18%.Conclusion The image quality of 80 kV scanning combined with AI optimization technique in CCTA of overweight patients is consistent with the standard 120 kV scanning with lower radiation dose,which can be promoted in clinical application.
