乳腺X线检查与磁共振弥散加权成像及动态增强扫描联合诊断乳腺占位性病变的价值分析

Value of breast X-ray examination combined with magnetic resonance diffusion weighted imaging and dynamic contrast-enhanced scanning in the diagnosis of breast space occupying lesions

目的分析乳腺X线检查与磁共振弥散加权成像(MR-DWI)及动态增强扫描(DCE)联合诊断乳腺占位性病变的临床价值。方法抽取2019年12月至2020年12月于濮阳市妇幼保健院初步诊断为乳腺占位性病变患者90例作为研究对象。纳入者均接受乳腺X线检查,磁共振弥散加权成像(MR-DWI)及动态增强扫描(DCE)技术联合检查,且后期接受病理诊断,以病理结果为依据,评价不同检查方案对病变性质的诊断效能。结果病理结果证实,90例患者中良性46例(51.11%),恶性44例(48.89%)。乳腺X线检查检出恶性38例,良性52例;MR-DWI及DCE技术联合检查检出恶性48例,良性42例;乳腺X线、MR-DWI及DCE技术联合检查检出恶性46例,良性44例。乳腺X线、MR-DWI及DCE技术联合检查的诊断准确度、特异度和敏感度均高于其单独检查(P均<0.05)。结论乳腺占位性病变临床选择乳腺X线检查、MR-DWI及DCE技术联合检查,在诊断特异度和敏感度方面表现优异,和单独乳腺X线检查相比,诊断准确性更高。

Objective To analyze the clinical diagnostic value of breast X-ray examination combined with magnetic resonance(MR)diffusion weighted imaging(DWI)and dynamic contrast-enhanced scanning(DCE)for breast space occupying lesions.Methods Ninety patients with breast space occupying lesions initially diagnosed in Puyang Maternal and Child Health Hospital from December 2019 to December 2020 were selected as the research objects.All enrolled patients underwent mammography combined with MR-DWI and DCE,and later received pathological diagnosis.Based on the pathological results,the diagnostic efficacy of different examination schemes for the nature of lesions were evaluated.Results The pathological results showed that 46 cases(51.11%)were benign,and 44 cases(48.89%)were malignant.Mammography detected 38 malignant cases and 52 benign cases;MR-DWI combined with DCE detected 48 malignant cases and 42 benign cases;combination examination of mammography,MR-DWI and DCE detected 46 malignant cases and 44 benign cases.The accuracy,specificity and sensitivity of combination examination of mammography,MR-DWI and DCE were higher than those of three single examinations(all P<0.05).Conclusions Combination examination of mammography,MR-DWI and DCE for breast space occupying lesions has excellent diagnostic specificity and sensitivity.Compared with mammography alone,the diagnostic accuracy of combination examination is higher.
