黄芪益肾方治疗慢性肾功能衰竭的临床效果

Effect of Huangqi Yishen decoction on chronic renal failure

目的探讨黄芪益肾方治疗慢性肾功能衰竭(CRF)的临床效果。方法抽取2019年6月至2021年3月于山西省中医院门诊就诊和住院的CRF患者80例,以随机数字表法分为对照组和治疗组,每组40例。对照组采用常规西医治疗,治疗组在对照组基础上加用黄芪益肾方治疗。比较两组中医证候积分、实验室指标、血清缺氧诱导因子1(HIF-1)和血小板反应蛋白1(TSP-1)水平。结果治疗后,治疗组中医证候积分[(9.08±2.77)分]低于对照组[(13.71±2.32)分],差异有统计学意义(P<0.01)。治疗前,两组实验室指标比较差异均未见统计学意义(P均>0.05);治疗后,治疗组血浆白蛋白、肾小球滤过率高于对照组,而血肌酐、尿素氮、尿酸、甲状旁腺素水平低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血清HIF-1、TSP-1水平比较差异均未见统计学意义(P均>0.05);治疗后,治疗组HIF-1水平高于对照组,TSP-1水平低于对照组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论黄芪益肾方治疗CRF,能提高患者的临床疗效,改善其血清HIF-1、TSP-1水平,安全性高。

Objective To investigate the clinical effect of Huangqi Yishen decoction in the treatment of chronic renal failure(CRF).Methods A total of 80 CRF patients were selected from outpatients and inpatients of Traditional Chinese Medicine Hospital of Shanxi from June 2019 to March 2021.They were divided into control group and treatment group by random number table method,with 40 cases in each group.The control group was treated with conventional western medicine,and the treatment group was treated with Huangqi Yishen decoction based on the treatment of the control group.The Traditional Chinese Medicine(TCM)syndrome score,laboratory indicators and levels of serum hypoxia-inducible factor 1(HIF-1)and thrombospondin-1(TSP-1)were compared between the two groups.Results After treatment,the TCM syndrome score in the treatment group(9.08±2.77)was lower than that in the control group(13.71±2.32),and the difference was statistically significant(P<0.01).Before treatment,there was no significant difference in laboratory indicators between the two groups(all P>0.05);after treatment,the albumin and estimated glomerular filtration rate in the treatment group were higher than those in the control group,while serum creatinine,blood urea nitrogen,uric acid and parathyroid hormone of the treatment group were lower than those of the control group(P<0.05).Before treatment,there was no significant difference in the levels of serum HIF-1 and TSP-1 between the two groups(all P>0.05);after treatment,HIF-1 in the treatment group was higher than that in the control group,and the level of TSP-1 was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Huangqi Yishen decoction in the treatment of CRF can improve the clinical efficacy,and improve the levels of serum HIF-1 and TSP-1,with high safety.
