胰腺中段切除术治疗良性及交界性肿瘤的效果及预后分析

Effect and prognosis of mid-pancreatectomy for benign and borderline pancreatic tumors

目的 对胰腺中段切除术治疗胰腺良性及交界性肿瘤的临床疗效进行分析。方法 回顾性分析2009年11月至2021年11月在安徽省立医院接受胰腺中段切除术的28例患者的病例资料并随访。结果 28例患者行胰腺中段切除+胰肠Roux-en-Y吻合,手术时间为(347.8±87.4)min,术中出血为(214.3±164.9)ml, 4例术中输血,28例患者均顺利完成手术,所有患者的病理诊断与术前相符,均为良性及交界性肿瘤。术后并发症方面,12例出现胰瘘症状,其中生化瘘和B级瘘各6例,1例术后出血行二次手术止血,9例腹腔感染,2例术后空腹血糖受损,1例术后糖尿病,所有患者术后消化吸收功能均无明显变化。结论 应用胰腺中段切除术治疗胰腺良性及交界性肿瘤,具有良好的效果,对患者胰腺内、外分泌功能具有显著保存功能。

Objective To study and analyze the clinical efficacy of mid-pancreatectomy(MP) for benign and borderline tumors of the pancreas. Methods The case data of 28 patients who underwent MP in Anhui Provincial Hospital from November 2009 to November 2021 were retrospectively analyzed and followed up. Results 28 patients underwent MP combined with pancreatic-intestinal Roux-en-Y anastomosis. The operative time was(347.8±87.4) min, and the intraoperative bleeding was(214.3±164.9) ml. There were 4 cases of blood transfusion during the operation. The operations were successfully completed in 28 patients, and the pathological diagnosis in all patients was the same as the preoperative diagnosis. All tumors were benign and junctional. In terms of postoperative complications, 12 cases developed pancreatic fistulae, including 6 cases of biochemical leakage and 6 cases of grade B leakage. 1 case developed postoperative bleeding and a second operation was performed to stop the bleeding. 9 cases developed abdominal infection. 2 cases developed impaired fasting glucose postoperatively. One case developed diabetes after operation, and all patients had no obvious impairment of digestive and absorption function after operation. Conclusions The application of mid-pancreatectomy in the treatment of benign and borderline tumors of the pancreas has a good effect and has a significant function of preserving the endocrine and exocrine functions of the pancreas.
