三阴性乳腺癌的声像学特征研究

Study on the sonographic characteristics of triple-nefative breast cancer

目的对比分析三阴性乳腺癌(TNBC)和非三阴性乳腺癌(Non-TNBC)的声像学特征,以筛选高频超声对TNBC的鉴别指标。方法回顾性分析免疫组织化学分型确诊的137例TNBC和82例Non-TNBC患者的临床病理资料和高频超声相关特征,有序多分类logistic回归分析超声指标与TNBC诊断的相关性。结果相比较于Non-TNBC,TNBC患者在年龄(<50岁)、肿瘤大小(≥2 cm)、组织学分级、淋巴结转移等,差异均有统计学意义(P<0.05);超声显示TNBC的病变边界倾向于具有突变的界面、肿块大部分呈后方回声增强,Non-TNBC边缘通常呈角状或尖锐状,肿块呈后方回声少部分增强。边缘不光整、后方回声增强和多腋窝LN与TNBC具有相关性。结论高频超声检查有助于早期识别TNBC。

Objective To compare and analyze the sonographic characteristics of triple-negative breast cancer(TNBC)and non-triple-negative breast cancer(Non-TNBC)in order to screen the identification parameters of TNBC by using the high-frequency ultrasound.Methods The clinicopathological data and high-frequency ultrasound-related features of 137 patients with TNBC and 82 patients with Non-TNBC diagnosed by immunohistochemical classification were retrospectively analyzed.Ordered multi-class logistic regression was used to analyze the correlation between ultrasound parameters and the diagnosis of TNBC.Results Compared with the non-TNBC group,the TNBC group was significantly different in age(<50 years),tumor size(≥2 cm),histological grade and lymph node metastasis(P<0.05).The ultrasound showed that the lesion boundary of TNBC tended to have mutated interfaces and most of the masses showed posterior echo enhancement.The non-TNBC edges were usually angular or sharp,and the masses showed a small part of posterior echo enhancement.Unsmooth edges,enhanced the posterior echo,and multiple axillary LN were correlated with TNBC.Conclusion High-frequency ultrasound examination is useful for early identification of TNBC.
