2型糖尿病患者血浆TLR-2水平变化及其临床意义

Changes of plasma TLR-2 levels in patients with type 2 diabetes mellitus and its clinical significance

目的探讨血浆TLR-2水平在2型糖尿病肾病患者的变化。方法将142例研究对象分为单纯糖尿病组(DM组)36例、糖尿病肾病组(DN组)106例,DN组根据尿白蛋白分为:正常白蛋白尿组(DN1组)35例、微量白蛋白尿组(DN2组)38例、大量白蛋白尿组(DN3组)33例。检测血清肌酐与血糖等生化指标及TLR-2水平,测定24 h尿白蛋白,比较各组间的差异。结果DN组的TLR-2水平均高于DM组(P<0.05);DN组的DN3、DN2、DN13组间(TLR-2)水平显著不同且随尿白蛋白增加而升高(P<0.05);TLR-2水平与糖尿病病程、BMI、UAER、TG呈正相关性,与HDL呈负相关性。结论2型糖尿病肾病发生发展与TLR-2受体激活密切相关。

Objective To investigate the changes of TLR-2 in patients with type 2 diabetic nephropathy.Methods 142 patients were divided into simple diabetes group(DM group,n=36)and diabetic nephropathy group(DN group,n=106).DN group was divided into normoalbuminuria group(DN1 group,n=35),microalbuminuria group(DN2 group,n=38),macroalbuminuria group(DN3 group,n=33)according to urinary albumin.Serum creatinine,blood glucose and TLR-2 levels were measured,24-hour urinary albumin was measured,and the differences between the groups were compared.Results The levels of TLR-2 in DN group were higher than those in DM group(P<0.05).TLR-2 was significantly different among DN3,DN2,and DN1 groups and increased with the increase of urinary albumin(P<0.05).The levels of TLR-2 were positively correlated with the duration of diabetes,BMI,UAER,and TG.HDL was negatively correlated.Conclusion The occurrence and development of type 2 diabetic nephropathy are closely related to the activation of TLR-2 receptor.
