基于数据挖掘探讨穴位疗法治疗甲状腺功能减退症临床选穴规律

Analysis of Clinical Point Selection Rules of Acupoint Therapy For Hypothyroidism Based on Data Mining

目的:基于数据挖掘技术探讨穴位疗法治疗甲状腺功能减退症(简称甲减)的临床选穴规律,为临床运用穴位疗法治疗甲减提供思路。方法:阅读筛选维普、万方、知网、Sino Med、Pubmed等数据库中自建库以来至2021年5月12日的运用穴位疗法治疗甲减的相关文献,应用Excel软件统计所涉穴位处方,对文献类型、辨证分析、干预方法,以及腧穴使用频次、分布、归经和特定穴种类进行总结,运用SPSSModeler18及SPSSStatistics25.0软件对高频腧穴进行关联规则与聚类分析。结果:共筛选出目标文献36篇,并提取出53组腧穴处方。统计分析得出治疗甲减使用频次前5位的腧穴是肾俞、足三里、脾俞、关元、气海;腧穴分布以四肢和腰背部为主;最常选取的经脉为膀胱经;使用最多的特定穴为背俞穴;关联规则显示:核心穴对为“肾俞-足三里”,核心腧穴处方为“肾俞、足三里、脾俞、气海”;聚类分析共有4组有效聚类群。结论:穴位疗法治疗甲减在施治方式及腧穴经络选择上存在一定规律性,治疗以温阳补气为要,艾灸疗法使用最广。

Objective:Using data mining technology to analyze the clinical acupoint selection rules of applying acupoint therapy to treat hypothyroidism,and provide ideas for the clinical application of acupoint therapy to treat hypothyroidism.Methods:Reading and screening the literature on acupoint therapy for hypothyroidism in related databases(including VIP,WANFANG,CNKI,SinoMed,Pubmed)from the establishment of the database to May 12,2021,and using Excel software to calculate the involved for all the prescriptions of acupoints received,summarize the document types,syndrome differentiation analysis,intervention methods,frequency,distribution,meridian and specific acupoints,and use SPSS Modeler18 and SPSS Statistics 25.0 to conduct association rules and cluster analysis of high-frequency acupoints.Results:A total of 36 target documents were selected out,and 53 groups of acupoint prescriptions were extracted.Descriptive analysis showed that the five acupoints with the highest frequency of acupuncture treatment of Hypothyroidism were Shenshu(BL 23)、Zusanli(ST 36)、Pishu(BL 20)、Guanyuan(RN 4)、Qihai(RN 6).The bladder meridian is the most commonly utilized meridian.The distribution of acupuncture points is mainly on the limbs and back.Basing on the data mining technology,the core acupoint pair for acupuncture treatment of hypothyroidism is Shenshu(BL 23)and Zusanli(ST 36),and the core acupoint prescriptions are"Shenshu(BL 23),Zusanli(ST 36),Pishu(BL 20)and Qihai(RN6)".The cluster analysis contains a total of 4 effective clusters.Conclusion:Acupoint therapy in the treatment of hypothyroidism has certain regularity in the treatment methods and the selection of acupoints and meridians.This research can provide some ideas for the basic research and clinical treatment of acupoint therapy for hypothyroidism.
