脉冲式冲洗尿管对尿道下裂术后并发症的影响

Effect of pulsed flushing urinary catheter on postoperative complications of hypospadias

目的探讨尿道下裂术后运用定期温热生理盐水脉冲式冲洗尿管的护理对术后并发症的影响,为减少尿道下裂术后患者并发症提供参考方案。方法选取2018年9月—2019年9月在新疆医科大学第一附属医院确诊并接受尿道下裂修复术的279例尿道下裂患儿,按照随机数字表法分为对照组139例和干预组140例。对照组采用常规护理方式,干预组在常规护理方式基础上,于术后第3天开始温热生理盐水脉冲式冲洗尿管,1次/d,持续直至拔除尿管。观察患儿拔除尿管后并发症发生率,术后出血量及留置尿管时间。结果干预组并发症发生率为8.89%,低于对照组的34.07%,差异有统计学意义(P<0.05)。干预组术后出血量为(9.83±3.52)ml,少于对照组的(12.81±4.57)ml,留置尿管时间为(8.45±2.22)d,短于对照组的(10.54±2.42)d,差异均有统计学意义(P<0.05)。结论定时温热生理盐水脉冲式冲洗尿管可减少尿道下裂术后并发症的发生,并能加快患儿的术后康复,值得临床推广应用。

Objective To explore the effect of regular warm normal saline pulsed flushing urinary catheter on postoperative complications after hypospadias operation,in order to provide reference scheme for reducing the postoperative complications of hypospadias.Methods A total of 279 children with hypospadias who were diagnosed in First Affiliated Hospital of Xinjiang Medical University from September 2018 to September 2019 and underwent repair of hypospadias were divided into 139 cases in the control group and 140 cases in the intervention group according to random number table method.The control group was treated with routine nursing,and the intervention group,on the basis of routine nursing,began to flush the urinary catheter with warm normal saline pulsed once a day on the 3rd day after operation,and continued until the urinary catheter was removed.The incidence of complications,the amount of bleeding and the time of indwelling catheter of children were observed between two groups after pulling out the urinary catheter.Results The incidence of complications in the intervention group was 8.89%,which was lower than 34.07%in the control group,and the difference was statistically significant(P<0.05).The amount of postoperative bleeding in the intervention group was(9.83±3.52)ml,which was less than(12.81±4.57)ml in the control group,and the indwelling catheter time was(8.45±2.22)d,which was shorter than(10.54±2.42)d in the control group,and the differences were statistically significant(P<0.05).Conclusion Regular warm normal saline pulsed flushing urinary catheter could reduce the occurrence of postoperative complications of hypospadias and accelerate postoperative recovery of children,which was worthy of clinical promotion and application.
