摘要
目的分析椎管内分娩镇痛产妇体温与产程中白细胞计数(WBC)、C-反应蛋白(CRP)水平变化的关系。方法选取2019年1—12月于肇庆市端州区妇幼保健院进行分娩的产妇80例为研究对象,根据不同镇痛方法将患者分为椎管内分娩镇痛组35例和无椎管内分娩镇痛组45例,统计两组产妇发热情况和各时段的平均体温,并比较两组产妇WBC、CRP水平变化。结果与无椎管内分娩镇痛组比较,椎管内分娩镇痛组发热率明显升高,差异有统计学意义(P<0.05)。两组产妇平均体温均先升高后降低,且入院时、产后24 h两组产妇平均体温,差异无统计学意义(P>0.05);而产后4 h,与无椎管内分娩镇痛组比较,椎管内分娩镇痛组平均体温明显升高,差异有统计学意义(P<0.05)。与产前比较,产后24 h两组产妇WBC、CRP水平均升高,差异均有统计学意义(P<0.05)。但产前、产后24 h,椎管内分娩镇痛组产妇WBC、CRP水平与无椎管内分娩镇痛组比较,差异无统计学意义(P>0.05)。结论椎管内分娩镇痛是导致产妇分娩时发热的因素,但产妇体温变化与感染情况无关。
Objective To analyze the relationship between maternal body temperature and the changes of white blood cell count(WBC)and C-reactive protein(CRP)during labor.Methods A total of 80 pregnant women who delivered in Zhaoqing Duanzhou District Maternal and Child Health Hospital from January to December 2019 were selected as the research objects.According to different analgesia methods,the patients were divided into the intraspinal delivery analgesia group with 35 cases and the non intraspinal delivery analgesia group with 45 cases.The fever and average body temperature of each period in the two groups were counted,and the changes of WBC and CRP levels of the two groups were compared.Results Compared with the non intraspinal delivery analgesia group,the fever rate in the intraspinal delivery analgesia group was significantly higher,and the difference was statistically significant(P<0.05).The average body temperature of the two groups of pregnant women first increased and then decreased,and there was no statistically significant difference in the average body temperature between the two groups at the time of admission and 24 hours after delivery(P>0.05).At 4 hours postpartum,compared with the non intraspinal delivery analgesia group,the average body temperature of the intraspinal delivery analgesia group increased significantly,and the difference was statistically significant(P<0.05).Compared with prenatal,the levels of WBC and CRP in the two groups at 24 hours after delivery were increased,and the differences were statistically significant(P<0.05);however,there were no statistically significant differences in WBC and CRP levels between the two groups before delivery and 24 hours after delivery(P>0.05).Conclusion Intraspinal delivery analgesia was a factor leading to maternal fever during delivery,but maternal body temperature changes had nothing to do with infection.
