增加预冲周期应用于血液灌流重症中毒患者的效果

Effect of increasing pre-flushing cycle applied to severely poisoned patients with hemoperfusion

目的探讨增加预冲周期应用于血液灌流(HP)重症中毒患者的效果。方法选取2019年4月至2021年6月于南华大学衡阳医学院附属第二医院就诊的92例HP重症中毒患者作为研究对象,按照随机数字表法分为对照组和研究组,每组46例。对照组给予静态预冲法,研究组给予动态预冲法,比较两组患者凝血相关指标及出血情况。结果灌注后,研究组凝血活酶时间(APTT)、凝血酶原时间(PT)短于对照组,纤维蛋白原(Fib)、血小板计数(PLT)高于对照组,差异有统计学意义(P<0.05)。研究组出血发生率(2.17%)低于对照组(17.39%),差异有统计学意义(P<0.05)。结论HP重症中毒患者增加预冲周期可减少灌流后血小板丢失,进而降低对凝血功能的影响,减少穿刺口出血情况发生。

Objective To explore the effect of increasing the pre-flushing cycle applied to severely poisoned patients with hemoperfusion(HP).Methods A total of 92 patients with severe HP poisoning admitted to the Second Affiliated Hospital of Hengyang Medical College of Nanhua University from April 2019 to June 2021 were selected as research objects.According to the random number table method,they were divided into control group and study group,with 46 cases in each group.The control group was given a static pre-filling method,and the study group was given a dynamic pre-filling method,compare the coagulation-related indicators and bleeding between the two groups.Results After perfusion,the activated partial thromboplastin time(APTT)and prothrombin time(PT)of the study group were shorter than those of the control group,and the fibrinogen(Fib)and platelet count(PLT)were higher than those of the control group,the differences were statistically significant(P<0.05).The incidence of bleeding in the study group(2.17%)was lower than that in the control group(17.39%),and the difference was statistically significant(P<0.05).Conclusion In patients with severe HP poisoning,increasing the pre-flushing cycle can reduce platelet loss after perfusion,thereby reducing the impact on coagulation function and the occurrence of bleeding at the puncture port.
