功能社区家庭医生签约服务实践与反思

Practice and Reflection on Contracted Service of Family Doctors in Functional Communities

目的:剖析上海市长宁区虹桥街道功能社区家庭医生签约服务的具体做法,总结经验并为同行提供借鉴。方法:通过文献研究和对辖区商务楼宇的典型抽样调研,明确辖区内功能社区人群需求特点。于2020年4月—2021年6月,面向辖区内功能社区人群分别开展了体验式服务、组团式服务和驻点式服务3种模式的实践探索。结果:体验式服务共开展12次,接受健康教育者共465人次,成功签约12人;组团式服务共开展6次,接受健康教育者共465人次,有效签约23人;驻点式服务中,共建立健康档案2012份,有效签约20人。结论:对于功能社区人群,体验式服务具有较好的宣传效果,但驻点式服务在改善健康认知和健康管理效果方面更能发挥作用;为功能社区人群提供更为个性化的健康服务,则需要及时引入商业保险。

OBJECTIVE To analyze the specific practices of family doctors signing services in the functional community of Shanghai Changning District Hongqiao Street,to summarizes experience and provides reference fo rthe counterparts in the practice of functional community services.METHODS Through literature research and a typical sample survey of commercial buildings in the ju⁃risdiction,demand characteristics of the population in functional communities in the jurisdiction were clarified,from April 2020 to June 2021,Shanghai Changning district Hongqiao street community health service center,carried out three modes of practice explora⁃tion,including experiential services,group-based services,and site-based services for people in functional communities,and timely recorded the service content and the number of people served.RESULTS A total of 12 experiential services were carried out and a to⁃tal of 465 people participated in health education,through which 12 people were finally successfully signed.In the model of group-based services,6 group services were carried out,and a total of 154 people participated in health education,through which 23 people were finally successfully signed.In the site-based service,2012 health files established.Through this model,20 people were effectively signed.CONCLUSION For people in functional communities,experiential services have a better publicity effect,but site-based services can play a better role in improving health awareness and health management effects.The health file establish⁃ment can lay a good foundation for continuous health management.
