Signing Status,Influencing Factors and Service Needs of Family Doctors in Functional Community Occupational Group

目的:分析上海市金山区功能社区职业人群家庭医生签约状况及影响因素,探索符合其需求的家庭医生服务模式。方法:采用多阶段分层整群抽样方法,对上海市金山区功能社区18-60岁职工开展问卷调查,了解该人群家庭医生签约状况及健康服务需求。结果:该人群家庭医生签约率为20.9%,家庭医生知晓率为29.5%,对签约服务内容需求最多的为健康医疗咨询(46.4%),最易接受的服务形式为微信咨询(38.5%),性别、年龄、岗位类型、学历、对家庭医生签约服务了解程度、优先就诊医院类型等6个因素对职业人群签约有影响。结论:职业人群家庭医生签约率较低,对签约服务内容和形式需求与社区签约居民有所不同,对职业人群应采取个性化的签约服务模式和健康管理方式。

OBJECTIVE To analyze the current status of family doctor contracting and its influencing factors among the occupational group in functional communities,to explore the family doctor service model that suits them,and to promote the health level of the group.METHODS A multi-stage stratified cluster sampling method was used to select 474 residences of functional communities in Jinshan District,and a questionnaire survey was conducted among employees aged 18 to 60 to analyze the current contract status and health service needs of the group.RESULTS The contracting rate of functional community population was 20.9%,and the awareness rate of family doctors was 29.5%;health and medical consultation(46.4%)had the most demand for contracted service content;the most acceptable form of service is WeChat consultation(38.5%);six factors,including gender,age,and occupation type,have an impact on whether occupational groups sign contracts.CONCLUSION The signing rate of the group is relatively low,and the content and form of signing services are different from those of daily signing residents.A personalized signing service model and a more comprehensive health management model should be adopted.
