超声引导双J管预置术对盆腔肿瘤治疗中输尿管损伤的预防作用

Preventive effect of ultrasound-guided double J tube presetting on ureteral injury in pelvic tumor treatment

目的探讨超声引导双J管预置术对盆腔肿瘤治疗中输尿管损伤的预防作用。方法回顾性分析2019年11月至2020年12月佛山市三水区人民医院收治的80例盆腔肿瘤患者的就诊情况,按照术前不同置管方式分为对照组(42例,接受常规置管)和预置组(38例,接受超声引导双侧双J管预置术),比较两组患者的手术情况,并发症和肾功能情况。结果预置组患者的手术时间为(179.32±24.19)min,明显短于对照组的(196.84±27.36)min,差异有统计学意义(P<0.05);预置组患者术中出血量、尿管留置时间分别为(117.65±13.42)mL、(123.09±11.29)d,与对照组的(123.09±11.29)mL、(13.54±1.42)d比较差异均无统计学意义(P>0.05);预置组患者的输尿管损伤率为0,明显低于对照组的14.29%,差异有统计学意义(P<0.05);预置组患者的尿潴留、泌尿系感染发生率分别为7.89%、5.26%,与对照组的4.76%、0比较差异均无统计学意义(P>0.05);两组患者术前和术后的肾功能比较差异均无统计学意义(P>0.05)。结论超声引导双J管预置术能够有效预防盆腔肿瘤治疗中的输尿管损伤,且未增加术后并发症的发生。

Objective To explore the preventive effect of ultrasound-guided double J tube presetting on ureter-al injury in pelvic tumor treatment.Methods A retrospective analysis was made on the treatment of 80 patients with pelvic tumors admitted to Foshan Sanshui District People's Hospital from November 2019 to December 2020.According to different preoperative catheterization methods,they were divided into control group(42 cases,received conventional catheterization)and presetting group(38 cases,received ultrasound-guided bilateral double-J tube presetting).The opera-tion,complications,and renal function of the two groups were compared.Results The operation time of patients in the presetting group was(179.32±24.19)min,which was significantly shorter than(196.84±27.36)min in the control group(P<0.05).The amount of bleeding during operation and the time of indwelling catheter in the presetting group were(117.65±13.42)mL and(123.09±11.29)d,which were not significantly different from(123.09±11.29)mL and(13.54±1.42)d in the control group(P>0.05).The ureteral injury rate in the presetting group was 0,which was significantly low-er than 14.29%in the control group(P<0.05).The incidence of urinary retention and urinary tract infection in the pre-setting group was 7.89%and 5.26%,which was not significantly different from 4.76%and 0 in the control group(P>0.05).There was no significant difference in renal function between the two groups before and after operation(P>0.05).Conclusion Ultrasound-guided double J tube presetting can effectively prevent ureteral injury in pelvic tumor treatment without increasing postoperative complications.
