探讨经阴道四维子宫输卵管超声造影技术在女性不孕症诊断及治疗中的应用价值

To investigate the clinical value of transvaginal four-dimensional hysterosalpingo-contrast sonography(TV 4D-HyCoSy)in the diagnosis and treatment of female infertility

目的:探讨经阴道四维子宫输卵管超声造影技术在女性不孕症诊断及治疗中的应用价值。方法:回顾性分析2020年4月—2022年2月贵州中医药大学第二附属医院及贵阳市妇幼保健院因不孕不育而行宫腹腔镜输卵管探查及四维子宫输卵管超声造影检查的34例患者作为研究对象。以宫腹腔镜下输卵管通液诊断结果作为金标准,分析经阴道四维子宫输卵管超声造影检查的准确性及在输卵管通畅性评价中的应用价值。结果:经阴道四维子宫输卵管超声造影检查时间、检查后下床活动时间及检查后疼痛视觉模拟评分(VAS)评分均明显低于宫腹腔镜检查,差异有统计学意义(P<0.05)。经阴道四维子宫输卵管超声造影对输卵管通畅诊断符合率为83.33%(10/12),对输卵管通而不畅诊断符合率为80.00%(8/10),对输卵管不通的诊断符合率为83.33%(10/12),诊断总符合率为82.35%(28/34)。结论:经阴道四维子宫输卵管超声造影检查能够动态、清晰的显示子宫宫腔以及输卵管的解剖结构,准确评价输卵管的通畅性,可作为一种简单、易行、安全、无创的不孕症辅助诊断方法,具有临床应用及推广价值。

Objective To explore the value of transvaginal four-dimensional hysterosalpingo-contrast sonography(TV 4D-HyCoSy)in the diagnosis and treatment of female infertility.Methods 34 patients who underwent hysteroscopic fallopian tube exploration and four-dimensional TV 4D-HyCoSy due to infertility in the Second Affiliated Hospital of Guizhou University of traditional Chinese medicine and Guiyang maternal and child health hospital from April 2020 to February 2022 were analyzed retrospectively.The accuracy of TV 4D-HyCoSy and its application value in the evaluation of tubal patency were analyzed.Results The time of TV 4D-HyCoSy,the time of getting out of bed after examination and the VAS score after examination were significantly lower than those of hysteroscopy(P<0.05).In the examination results,the coincidence rate of TV 4D-HyCoSy in the diagnosis of tubal patency was 83.33%(10/12),80.00%(8/10),and 83.33%(10/12),The total coincidence rate of diagnosis was 82.35%(28/34).Conclusion Transvaginal four-dimensional hysterosalpingo-contrast sonography(TV 4D-HyCoSy)can be used as a simple,easy,safe and noninvasive auxiliary diagnosis method of infertility.It can dynamically and clearly display the anatomical structure of uterine cavity and fallopian tube,and accurately evaluate the patency of fallopian tube.
