PCNE分类系统在血液科医药联合门诊中的实践探索

Practical exploration of pharmaceutical care network Europe Classification System in physician-pharmacist joint clinic of hematology

目的 探讨欧洲医药保健网(PCNE)分类系统在血液科医药联合门诊药物相关问题(DRPs)中的作用。方法 采用PCNE(9.0版)分类系统对血液科医药联合门诊3个病例实施药学监护,对发生的DRPs的类别、原因、干预、干预接受程度以及DRPs最终状态等进行分析。结果 3个病例各发生DRPs 1~2个,选药不适宜、不适当的组合和患者用药不当是导致DRPs的主要原因。临床药师借助PCNE分类系统进行系统地分析及干预,DRPs得到全部或部分解决。结论 PCNE分类系统适用于血液科医药联合门诊的药学监护。PCNE分类系统有助于实现药学监护模式的快捷化、标准化和精准化,提高用药合理性,保障患者用药安全性。

Objective To explore the role of Pharmaceutical Care Network Europe(PCNE)classification system in drug-related problems(DRPs)in physician-pharmacist joint clinic of hematology.Methods The classification system(version 9.0)was used to perform the pharmaceutical care for three patients in physician-pharmacist joint clinic of hematology,and the types,causes,interventions,acceptance and the final situation of DRPs were analyzed.Results One to two DRPs occurred in the three cases.Inappropriate drug,inappropriate combination of drugs and inappropriate medication of patients were the main causes of DRPs.Clinical pharmacists used PCNE classification system to analyze and intervene DRPs systematically,and DRPs were totally or partially solved.Conclusion PCNE classification system is suitable for pharmaceutical care in physician-pharmacist joint clinic of hematology.PCNE system is helpful to realize the rapidness,standardization and precision of pharmaceutical care model,improve the rationality of drug use,and ensure the safety of medication for patients.
