股神经阻滞联合鸡尾酒疗法对膝关节置换术后患者的应用效果

Application Effect of Femoral Nerve Block Combined with Cocktail Therapy After Total Knee Replacement

目的:观察超声引导下股神经阻滞联合鸡尾酒疗法对全膝关节置换术后患者的应用效果,探讨其可行性和安全性。方法:选取2018年2月-2020年5月南宁市中医医院收治的全膝关节置换术患者70例,随机分为观察组和对照组,各35例。观察组接受超声引导下股神经阻滞联合鸡尾酒疗法,对照组接受超声引导下股神经阻滞。观察两组患者术后6 h、12 h、24 h、36 h疼痛视觉模拟评分(VAS评分),术后第2周、1个月、6个月膝关节功能评分(KSS评分)及不良反应发生率。结果:观察组术后6 h、12 h、24 h、36 h VAS评分均低于对照组,差异有统计学意义(P<0.05);两组术后2周KSS评分比较,差异无统计学意义(P>0.05)。观察组1个月、6个月KSS评分高于对照组,差异有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:超声引导下股神经阻滞联合鸡尾酒疗法可缓解全膝关节置换术后患者疼痛症状,改善早期的膝关节活动度,安全性高,值得推广应用。

Objective:To observe the application effect of femoral nerve block guided by ultrasound combined with cocktail therapy in postoperative patients after total knee replacement,and to explore its feasibility and safety.Methods:A total of 70patients undergoing total knee replacement in Nanning Hospital of Traditional Chinese Medicine from February 2018 to May 2020were selected and randomly divided into observation group and control group,with 35 cases in each group.The observation group received ultrasound-guided femoral nerve block combined with cocktail therapy,while the control group received ultrasoundguided femoral nerve block.Visual analogue scale(VAS) pain scores at 6 h,12 h,24 h and 36 h after surgery,knee joint function score(KSS score) at 2 w,1 m and 6 m after surgery and adverse reactions of patients in two groups were observed.Results:VAS pain scores at 6 h,12 h,24 h and 36 h after surgery in the observation group were lower than those in the control group,and the difference was statistically significant(P0.05).The KSS score of the observation group at 1 m and 6 m after surgery was higher than that of the control group,and the difference was statistically significant(P0.05).Conclusion:Ultrasound-guided femoral nerve block combined with cocktail therapy in patients after total knee replacement can relieve the early postoperative pain symptoms,improve the early range of motion of the knee joint,with high safety,it is worthy of promotion and application.
