N末端前体脑利钠肽和D-二聚体检测对心力衰竭的诊断价值

摘要 目的:分析N末端前体脑利钠肽(NT-proBNP)与D-二聚体(D-D)检测在心力衰竭诊断中的价值。方法:选取2020年1月-2021年9月北京市昌平区中西医结合医院收治的射血分数保留性心力衰竭患者98例设为观察组,并将同时间段内100例健康体检者设为对照组,回顾性分析两组资料,观察两组NT-proBNP、D-D水平差异,对比观察组中不同心功能分级患者的NT-proBNP、D-D水平,分析射血分数保留性心力衰竭的主要危险因素。结果:两组年龄、心功能分级、NT-proBNP及D-D水平比较,差异有统计学意义(P<0.05);高龄、高NT-proBNP水平以及高D-D水平均是射血分数保留性心力衰竭的主要危险因素,OR值均>1;观察组患者心功能分级升高,NT-proBNP及D-D水平相应升高,差异有统计学意义(P<0.05)。结论:NT-proBNP与D-D检测对心力衰竭有着较高诊断价值,高龄、高NT-proBNP水平以及高D-D水平是射血分数保留性心力衰竭的主要危险因素,且伴随心力衰竭程度加重,患者NT-proBNP与D-D水平相应升高,便于临床评定其心功能分级。 Objective:To analyze the value of N-terminal precursor brain natriuretic peptide(NT-pro BNP) and D-dimer(D-D)detection in the diagnosis of heart failure.Methods:A total of 98 patients with heart failure with preserved ejection fraction in theBeijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine,Mental Combined TuberculosisDistrict from January 2020 to September 2021 were included as the observation group,and 100 healthy examination subjects inthe same time period were included as the control group.A retrospective analysis of the clinical data in two groups was performed.The differences in the levels of NT-pro BNP and D-D between the two groups were observed.The levels of NT-pro BNP and D-Din patients with different cardiac function classifications in the observation group were compared.The main risk factors for heartfailure with preserved ejection fraction were analyzed.Results:There were statistically significant differences in age,cardiacfunction classification,NT-pro BNP and D-D levels between the two groups(P 1.In theobservation group,the levels of NT-pro BNP and D-D increased correspondingly with the increase of cardiac functionclassification,with statistically significant differences(P<0.05).Conclusion:The detection of NT-pro BNP and D-D has a highdiagnostic value for heart failure.Old age,high NT-pro BNP level and high D-D level are the main risk factors for heart failurewith preserved ejection fraction,and with the worsening of heart failure,the levels of NT-pro BNP and D-D are correspondinglyincreased,which is convenient for clinical evaluation of the cardiac function classification.
