尤瑞克林、丁苯酞联合高压氧对脑梗死急性期患者神经功能和血管内皮功能的影响 被引量:3

Effects of uricillin, butylphthalide and hyperbaric oxygen on nerve and vascular endothelial function in patients with acute cerebral infarction

摘要 目的 探讨尤瑞克林、丁苯酞联合高压氧对脑梗死急性期患者神经功能和血管内皮功能的影响。方法 回顾性分析2018年2月-2019年2月医院收治的94例脑梗死急性期患者临床资料,根据治疗方案分为高压氧组和联合组,各47例;高压氧组在常规治疗的基础上给予高压氧治疗,联合组在常规治疗的基础上给予尤瑞克林、丁苯酞联合高压氧治疗。对比治疗前、治疗1周、2周后2组患者神经功能缺损程度(NIHSS评分),并对比临床疗效;对比治疗前后2组患者血管内皮功能指标:血清一氧化氮(NO)、血浆内皮素-1(ET-1);统计治疗期间不良反应发生率。结果 NIHSS评分组间、时间、交互比较,差异有统计学意义(P<0.05),治疗前2组NIHSS评分比较,差异无统计学意义(P>0.05),治疗1周、2周后联合组NIHSS评分均低于高压氧组(P<0.05),随时间的延长2组NIHSS评分均呈降低趋势(P<0.05)。联合组和高压氧组临床疗效等级分布比较,差异有统计学意义(P<0.05),且联合组总有效率高于高压氧组(P<0.05)。治疗前血清NO、血浆ET-1比较,差异均无统计学意义(P>0.05);与治疗前比,治疗后2组血清NO水平均升高,血浆ET-1水平降低,差异均有统计学意义(P<0.05);与高压氧组比,联合组血清NO水平较高,血浆ET-1水平较低,差异均有统计学意义(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论 尤瑞克林、丁苯酞联合高压氧可显著改善脑梗死急性期患者神经功能和血管内皮功能,疗效显著,安全可靠。 Objective To investigate the effects of uricline,butylphthalide and hyperbaric oxygen on the function of neurological and vascular endothelial in patients with acute cerebral infarction.Methods The clinical data of 94 patients with acute cerebral infarction from February 2018 to February 2019 were analyzed retrospectively.According to the treatment plan,they were divided into hyperbaric oxygen group and combined group,47 cases each;The hyperbaric oxygen group was treated with hyperbaric oxygen on the basis of routine treatment,and the combined group was treated with uricline,butylphthalide and hyperbaric oxygen on the basis of routine treatment.The degree of neurological impairment(NIHSS score)was compared before treatment,1 week and 2 weeks after treatment,and the clinical efficacy was compared.Vascular endothelial function indexes:serum nitric oxide(NO)and plasma endothelin-1(ET-1)before and after treatment were compared between the two groups.The incidence of adverse reactions during treatment was counted.Results There were significant differences in NIHSS scores among groups,moments and interaction(P<0.05).There was no significant difference in NIHSS score between the two groups before treatment(P>0.05).The NIHSS score of thecombined group was lower than that of the hyperbaric oxygen group after 1 and 2 weeks of treatment(P<0.05),and the NIHSS score of the two groups decreased with time(P<0.05).There was significant difference in the clinical efficacy grade distribution between the combined group and the hyperbaric oxygen group(P<0.05),and the total effective rate of the combined group was higher than that of the hyperbaric oxygen group(P<0.05).There was no significant difference in serum NO and plasma ET-1 before treatment(P>0.05).Compared with before treatment,serum NO level increased and plasma ET-1 level decreased in both groups after treatment(P<0.05).Compared with the hyperbaric oxygen group,the combined group had higher serum NO level and lower plasma ET-1 level(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Ureklin and butylphthalide combined with hyperbaric oxygen can significantly improve the neurological and vascular endothelial function in patients with acute cerebral infarction.The effect is remarkable,safe and reliable.
