二维超声联合超声造影对甲状腺良恶性结节的诊断价值

Evaluation of two-dimensional ultrasound scoring combined with contrast-enhanced ultrasound in the diagnosis of benign and malignant thyroid nodules

摘要 目的探讨二维超声联合超声造影对甲状腺良恶性结节的诊断价值。方法本研究为回顾性研究,选取2020年12月至2021年12月北部战区总医院收治的150例甲状腺结节患者(216个结节)的临床资料。男56例,女94例,年龄(46.72±10.53)岁,年龄范围为20~74岁。所有患者均行二维超声和超声造影检查。以病理检查结果为"金标准",比较二维超声、超声造影、二维超声联合超声造影诊断甲状腺良恶性结节的效果。结果二维超声、超声造影及二维超声联合超声造影诊断良恶性结节的符合率分别为81.5%(176/216)、83.8%(181/216)、90.7%(196/216)。二维超声联合超声造影诊断良恶性甲状腺结节的特异度[90.3%(130/144)]高于二维超声[77.1%(111/144)]和超声造影[79.9%(115/144)],二维超声联合超声造影诊断良恶性甲状腺结节的准确度[90.7(196/216)]高于二维超声[81.5%(176/216)]和超声造影[83.8%(181/216)],差异均有统计学意义(P<0.05)。结论二维超声联合超声造影诊断甲状腺良恶性结节可提高诊断符合率和诊断效能,值得在临床推广应用。 Objective To evaluate the value of two-dimensional ultrasound scoring combined with contrast-enhanced ultrasound in the diagnosis of benign and malignant thyroid nodules.Methods This study was a retrospective study,clinical data of 150 patients(216 nodules)with thyroid nodules admitted to Northern Theater Command General Hospital from December 2020 to December 2021 were retrospectively analyzed.sThere were 56 males and 94 females,the age was(46.72±10.53)years old,ranged from 20 to 74 years old.All patients underwent two-dimensional ultrasound and contrast-enhanced ultrasound.The results of pathological examination were taken as the gold standard to compare the effects of two-dimensional ultrasound,contrast-enhanced ultrasound and two-dimensional ultrasound combined with contrast-enhanced ultrasound in the diagnosis of benign and malignant thyroid nodules.Results The coincidence rate of two-dimensional ultrasound scoring,contrast-enhanced ultrasound and two-dimensional ultrasound combined with contrast-enhanced ultrasound were 81.5%(176/216),83.8%(181/216)and 90.7%(196/216).The specificity of two-dimensional ultrasound combined with contrast-enhanced ultrasound in diagnosing benign and malignant thyroid nodules[90.3%(130/144)]was higher than those of two-dimensional ultrasound scoring[77.1%(111/144)]and contrast-enhanced ultrasound[79.9%(115/144)],the accuracy of two-dimensional ultrasound combined with contrast-enhanced ultrasound in diagnosing benign and malignant thyroid nodules[90.7(196/216)]was higher than those of two-dimensional ultrasound score[81.5%(176/216)]and contrast-enhanced ultrasound[83.8%(181/216)],and the differences were statistically significant(P<0.05).Conclusions Two-dimensional ultrasound combined with contrast-enhanced ultrasound in the diagnosis of benign and malignant thyroid nodules can improve the diagnostic coincidence rate and diagnostic efficiency,which is worthy of clinical application.
