超声弹性成像在甲状腺结节良恶性诊断中的价值

Value of ultrasound elastography in benign and malignant diagnosis of thyroid nodules

摘要 目的探讨超声弹性成像(UE)对甲状腺结节患者的诊断价值。方法本研究为回顾性研究,选取2016年3月至2021年12月北部战区总医院超声诊断科检查的96例甲状腺结节患者,男42例,女54例,年龄(41.2±9.6)岁,年龄范围为11~67岁。所有患者均在治疗前行甲状腺常规超声检查与UE检查,且均接受手术切除甲状腺结节病理学检查,结合病理结果,对比分析甲状腺常规超声检查与UE对甲状腺结节良恶性诊断的灵敏度、特异度、准确度。结果术后病理诊断良性结节44个,恶性结节68个。UE对甲状腺结节诊断的灵敏度[91.2%(62/68)]、特异度[38.6%(17/44)]及准确度[81.8%(36/44)]均高于常规超声检查[67.9%(76/112)、87.5%(98/112)、86.8%(59/68)],且常规超声检查与UE诊断效能的特异度和准确度比较,差异有统计学意义(P<0.05)。结论UE在诊断甲状腺结节的良恶性方面具有很高的临床价值。 Objective To investigate the diagnostic value of ultrasound elastography(UE)in thyroid nodules.Methods In this retrospective study,96 patients with thyroid nodules,42 males and 54 females,aged(41.2±9.6)years old,ranging from 11 to 67 years old,were selected from the department of ultrasonography of General Hospital of Northern Theater Command from March 2016 to December 2021.All patients underwent conventional thyroid ultrasound examination and UE examination before treatment,and all received surgical resection of thyroid nodules for pathological examination.Combined with the pathological results,the sensitivity,specificity and accuracy of conventional thyroid ultrasound and UE in benign and malignant diagnosis of thyroid nodules were compared and analyzed.Results Postoperative pathology diagnosed 44 benign nodules and 68 malignant nodules.UE had higher sensitivity[91.2%(62/68)],specificity[38.6%(17/44)]and accuracy[81.8%(36/44)]than conventional ultrasonography[67.9%(76/112),87.5%(98/112),86.8%(59/68)].There were statistically significant differences in specificity and accuracy between conventional ultrasound examination and UE diagnostic efficacy(P<0.05).Conclusions UE has high clinical value in diagnosing benign and malignant thyroid nodules.
