吸气肌训练联合有氧运动对慢性CHF患者心肺功能的影响 被引量:9

摘要 目的:研究吸气肌训练(IMT)联合有氧运动对慢性心衰(CHF)患者心肺功能的影响。方法:于我院治疗的100例CHF患者被随机均分为有氧运动组(常规治疗基础上接受有氧运动)和联合训练组(有氧运动组基础上接受IMT),两组均治疗12周。观察比较两组治疗前后第一秒用力呼气容积(FEV1)、用力肺活量(FVC)、最大通气量(MVV)、峰值摄氧量(PeakVO_(2))、二氧化碳通气当量(VE/VCO_(2))、每分钟通气量(VE)、运动时间、6min步行距离(6MWD)及明尼苏达心力衰竭生活质量量表(MLHFQ)评分。结果:与有氧运动组比较,治疗后,联合训练组MVV[(70.74±2.57)L比(71.72±2.00)L]、VE[(64.40±3.67)L比(69.58±4.85)L]、运动时长[(8.90±1.83)min比(9.80±1.55)min]升高更显著,MLHFQ评分[(49.10±3.15)分比(43.60±2.79)分]、VE/VCO_(2)[(24.38±1.83)比(21.80±1.03)]降低更显著(P<0.05或<0.01)。结论:IMT联合有氧运动可显著改善慢性心衰患者肺功能,提高生活质量,延长运动时间。 Objective:To study influence of inspiratory muscle training(IMT)combined aerobic exercise on cardiopulmonary function in patients with chronic heart failure(CHF).Methods:A total of 100 CHF patients treated in our hospital were randomly and equally divided into aerobic exercise group(received aerobic exercise based on routine treatment)and combined training group(received IMT based on aerobic exercise group),both groups were treated for 12 weeks.Forced expiratory volume in one second(FEV1),forced vital capacity(FVC),maximal ventilatory volume(MVV),peak oxygen uptake(PeakVO_(2)),carbon dioxide ventilation equivalent(VE/VCO_(2)),minute ventilation(VE),exercise time,6 min walking distance(6 MWD)and score of Minnesota living with heart failure questionnaire(MLHFQ)before and after treatment were observed and compared between two groups.Results:Compared with aerobic exercise group,after treatment,there were significant rise in MVV[(70.74±2.57)L vs.(71.72±2.00)L],VE[(64.40±3.67)L vs.(69.58±4.85)L]and exercise time[(8.90±1.83)min vs.(9.80±1.55)min],and significant reductions in score of MLHFQ[(49.10±3.15)scores vs.(43.60±2.79)scores]and VE/VCO_(2)[(24.38±1.83)vs.(21.80±1.03)]in combined training group(P<0.05 or<0.01).Conclusion:IMT combined aerobic exercise can significantly improve lung function,improve quality of life and prolong exercise time in patients with chronic heart failure.
