贵州省性病门诊就诊者艾滋病知识和行为现状及HIV感染影响因素分析

Analysis of AIDS Knowledge and Behavior and HIV Infection Influencing Factors of STD Outpatients in Guizhou Province

目的了解贵州省性病门诊就诊者的艾滋病知识知晓情况、行为现状和HIV感染状况,分析HIV感染的影响因素。方法2018年4月-2020年6月,收集贵州省各市州22所医院以性病症状为主诉到皮肤性病科就诊的患者,对其进行问卷调查和HIV抗体检测。结果共纳入5204例患者,男女比例为2.1:1,平均年龄为(37.8±13.3)岁,性病门诊就诊者艾滋病知识总知晓率为64.91%(3378/5204),HIV抗体阳性率为1.11%(58/5204),最近1个月发生过商业性行为、临时性行为和男男肛交性行为的占26.36%(1372/5204),每次使用安全套的比例分别为79.11%(375/474)、75.49%(844/1118)和53.70%(29/54)。多因素Logistic回归分析显示:年龄35~44岁(OR=0.20,95%CI:0.04~0.91,P=0.038)的HIV感染风险低于18~24岁,离婚/丧偶(OR=4.87,95%CI:1.38~17.20,P=0.014)、既往做过HIV检测(OR=3.07,95%CI:1.56~6.04,P=0.001)、既往1个月发生过男男肛交性行为(OR=6.98,95%CI:2.27~21.47,P=0.001)的就诊者HIV感染风险高于未婚、既往未做过HIV检测、最近1个月未发生肛交性行为的就诊者。结论贵州省性病门诊就诊者的艾滋病知识知晓率较低,高危性行为发生频率高、安全套使用率低,HIV阳性率高。年龄18~24岁、离婚或丧偶、既往做过HIV检测、最近1个月发生过男男肛交性行为是HIV感染的影响因素。

Objective To investigate AIDS knowledge,behavior status and HIV infection status of HIV infection in STD outpatients in Guizhou province,and analyze the influencing factors of HIV infection.Methods A questionnaire survey and HIV antibody detection were conducted among the patients who visited dermatovenereal diseases department of 22 hospitals in every city of Guizhou province from April 2018 to June 2020 with STD symptoms as the chief complaint.Results A total of 5204 patients were included,with a male to female ratio of 2.1:1,the average age was(37.8±13.3)years old.The total awareness rate of AIDS knowledge among STD outpatients was 64.91%(3378/5204),and the HIV antibody positive rate was 1.11%(58/5204).The 26.36%(1372/5204)had commercial sex,temporary sex or anal sex in the last month,and the 79.11%(375/474),75.49%(844/1118)and 53.70%(29/54)used condoms each time,respectively.Multivariate logistic regression analysis showed that aged 35-44 years(OR=0.20,95%CI:0.04~0.91,P=0.038)had a lower risk of HIV infection than 18-24 years.Divorced/widowed(OR=4.87,95%CI:1.38~17.20,P=0.014),who had been tested for HIV(OR=3.07,95%CI:1.56~6.04,P=0.001)and had anal sex with man in last month(OR=6.98,95%CI:2.27~21.47,P=0.001)was higher than those who were unmarried,had not been tested for HIV in the past,and had not anal sex in the last month.Conclusion The awareness rate of AIDS knowledge is low,the frequency of high-risk sexual behaviors is high,condom use rate is low and HIV positive rate is high among STD out-patients in Guizhou province.Aged 18-24 years,divorced or widowed,had been tested for HIV,and male to male anal sex in last month are the influencing factors for HIV infection.
