盘龙七片联合中医外治三联法治疗膝骨性关节炎肾虚血瘀证50例临床观察

Clinical observation on 50 cases of knee osteoarthritis with kidney⁃asthenia and blood stasissyndrome treated by Panlongqi Pian(盘龙七片) combined with triplex external treatment of TCM

目的 观察盘龙七片联合中医外治三联法治疗膝骨性关节炎肾虚血瘀证的临床疗效。方法 将100例膝骨性关节炎肾虚血瘀证患者采用随机数字表法分为治疗组和对照组,各50例。对照组采用中医外治三联法(中药熏敷、压腿锻炼、手法弹拨)治疗,治疗组在对照组治疗方法的基础上联合盘龙七片治疗。2组均以4周为1个疗程,治疗1个疗程后比较2组的临床疗效、不良反应发生情况,以及治疗前后西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分、膝关节活动度(ROM)评分。结果 治疗2,4周后,2组疼痛、僵硬、进行日常活动的难度评分及总评分均明显降低,膝关节屈伸主、被动活动度均明显升高,与同组治疗前比较差异有统计学意义(P<0.05或P<0.01),且治疗组治疗4周后疼痛、僵硬及总评分均明显低于对照组,治疗2,4周后的膝关节屈伸主、被动活动度均明显高于对照组,与对照组同期比较差异有统计学意义(P<0.05或P<0.01);对照组优良率为52.0%(26/50),治疗组为74.0%(37/50),2组比较差异有统计学意义(P<0.05);对照组治疗过程中出现患膝瘙痒1例,对症治疗后症状消失,治疗组无明显不良反应。结论 盘龙七片联合中医外治三联法治疗可明显改善膝骨性关节炎肾虚血瘀证患者的临床症状,疗效确切,且安全性高,值得临床推广应用。

Objective To observe the clinical efficacy of Panlongqi Pian(盘龙七片)combined with triplex external treatment of TCM for knee osteoarthritis(KO)with kidney⁃asthenia and blood stasis syndrome.Methods Totally 100 cases of KO with kidney⁃asthenia and blood stasis syndrome were divided into treatment group and con⁃trol group by random number table method,with 50 cases in each group.The control group uses the triplex external treatment of TCM(fumigation with Chinese materia medica,leg press exercise,springing and plucking manipula⁃tion),on this basis,the treatment group was combined with Panlongqi Pian.The 2 groups took 4 weeks as 1 course of treatment.After 1 course of treatment,the clinical efficacy,adverse reactions,Western Ontario and McMaster Uni⁃versity(WOMAC)osteoarthritis index score and knee joint range of motion(ROM)score were compared between the 2 groups before and after treatment.Results After 2 and 4 weeks of treatment,the pain,stiffness,difficulty score and total score of daily activities in the 2 groups were significantly reduced,and the active and passive ROM of knee flexion and extension were significantly increased,and the differences were statistically significant than those in the same group before treatment(P<0.05 or P<0.01). Moreover,the pain,stiffness and total score of the treatmentgroup after 4 weeks of treatment were significantly lower than those in the control group. After 2 and 4 weeks oftreatment,the active and passive ROM of knee flexion and extension of the treatment group were significantly higherthan those in the control group,and the difference was statistically significant as compared with the control group inthe same period(P<0.05 or P<0.01). The excellent and good rate was 52.0%(26/50) in the control group and74.0%(37/50) in the treatment group. There was statistically significant difference between the 2 groups(P <0.05);In the control group,there was 1 case of knee pruritus during treatment,and the symptoms disappeared aftersymptomatic treatment. There was no marked adverse reactions in the treatment group. Conclusion Panlongqi Piancombined with triplex external treatment of TCM for KO with kidney⁃asthenia and blood stasis syndrome canmarkedly alleviate the symptoms of the patients with definite efficacy and high safety,which deserves clinical promo⁃tion and application.
