非砷角化多发性鳞癌1例

A Case of Non-Arsenic Keratotic Multiple Squamous Cell Carcinoma

患者女,49岁,双手、双前臂皮疹伴瘙痒7年,加重3年。四肢远端见散在淡褐色及红色扁平丘疹、斑块、溃疡。皮肤镜下可见局灶性肾小球样血管、点状血管(左手背);见粗大血管及大量出血(左手腕);见红色假网状改变、多发毛囊角栓和玫瑰花瓣征(面部)。皮肤组织病理显示左手腕、左小指、右足跟提示鳞状细胞癌;左手背提示鲍温病。诊断为多发性鳞状细胞癌;鲍温病。

A 49-year-old female patient presented with plaque on both hands and forearms with itching for 7 years, and aggravation for 3 years.Physical examination showed scattered light brown and red flat papules, plaques, and ulcers on the distal extremities.Focal glomerular blood vessels and punctate blood vessels could be seen under dermoscopy(back of the left hand);thick blood vessels and heavy bleeding could be seen(left wrist);red pseudo-reticular changes, multiple follicular horn plugs and rose petals signs could be seen(face).Skin histopathology indicated that the left wrist, left little finger, and right heel suggested squamous cell carcinoma;the back of the left-hand suggested Bowen′s disease.Diagnosis: 1.Multiple squamous cell carcinoma 2.Bowen′ disease.
